Objectivecomparing the laboratory related indexes and clinical outcomes of women’s serum As Ab positive and negative under IVF or ICSI insemination, the assisted pregnancy outcomes on IVF/ICSI-ET were researched.MethodsThe clinical data of IVF/ICSI cycles was received in the reproductive medicine center in the first affiliated hospital of Hainan medical university from January 2016 to July 2020.According to the results whether As Ab was contained in the serum of women before assisted reproduction, cycles were divided to the serum As Ab positive group(the observation group)and the negative group(the control group)with a ratio of one to one.A total of 467 cycles were included in ts study. In IVF cycles,As Ab positive cycles and negative cycles were 158 cycles respectively,In ICSI cycles,As Ab positive cycles and negative cycles were 57 cycles respectively. And 37 cycles of As Ab positive patients in Half-ICSI cycles were researched. The clinical basic data, fertilization, embryo development and clinical outcome of patients in each group were considered.According to the binding site between serum As Ab and sperm in IVF cycles, the cycles were divided into three groups(the head positive group, the tail positive group and negative group),each group was under the ratio of one to one to one.The clinical basic data, fertilization, embryo development and clinical outcome of patients among the three groups were considered.Results1.In IVF cycles, there was no significant difference between serum As Ab positive and negative groups in fertilization rate, cleavage rate, normal fertilization rate, multiple pronucleus fertilization rate, normal fertilized egg cleavage rate, quality embryo rate in cleavage stage, transplantable blastocyst rate, quality blastocyst rate, implantation rate, clinical pregnancy rate.2.In ICSI cycles, there was no significant difference in normal fertilization rate, cleavage rate, high quality embryo rate, transplantable blastocyst rate, high quality blastocyst rate, implantation rate, clinical pregnancy rate and early abortion rate between serum As Ab positive and negative groups(P>0.05).3.Comparison outcomes of IVF and ICSI fertilization in Half-ICSI,the normal fertilization rate of serum As Ab positive cycles in ICSI was higher than IVF cycles(78.73% vs 65.85%)(P < 0.05).Comparing the cleavage rate of normal fertilized eggs, the rate of high-quality embryos at cleavage stage, the rate of transplantable blastocysts and the rate of high-quality blastocysts between the two groups, all were not significant difference(P>0.05).Removing the immature oocytes after fertilization in IVF group, there was no significant difference between IVF and ICSI in the normal fertilization rate(76.33% vs 78.73%)(P>0.05).4.In IVF fertilization schemes, there was statistically significant between As Ab head positive group and negative group in the fertilization rate, the normal fertilization rate and the multiple-pronucleus fertilization rate(p < 0.05).Compared the As Ab head positive group with the As Ab tail positive group, the difference statistically significant in fertilization rate(p < 0.05).Compared the negative group and the As Ab tail positive group, there were no significant differences in the normal fertilization rate and the rate of multiple pronucleus fertilization and unfertilized rate(P>0.05).Comparing the unfertilized rate among the three groups, the difference was statistically significant(P<0.05),and the rate of As Ab head positive group was higher than the other two groups’.Comparing the cleavage rate of normal fertilized eggs, the rate of high-quality embryos in cleavage stage, the rate of transplanted blastocysts, the rate of high-quality blastocysts, the rate of clinical pregnancy and the rate of early abortion among the three groups, there were no significant difference(P>0.05).Conclusion1.As Ab Serum positive in women has no effect on the clinical outcome of IVF/ICSI-ET.2.For the female with As Ab positive, the mature oocytes were treated with IVF and ICSI,and the fertilization outcomes were similar.3.As Ab head positive in female serum may reduce fertilization rate in IVF.
