Objective:To study the clinical effects of transurethral plasma bipolar enucleation of prostate and transurethral plasma bipolar resection of prostate in the treatment of benign prostatic hyperplasia with a volume greater than 80 ml,and provide a basis for urologists to choose large-volume BPH surgery.Methods:A retrospective analysis of the clinical data of 89 patients who were hospitalized with BPH in our hospital and whose gland volume was more than 80 ml underwent surgical treatment was retrospectively analyzed,from January 2018 to December 2020.According to different surgical procedures,89 patients with BPH were divided into two groups,39 cases in enucleation group(receiving TUKEP),and50 cases in electrotomy group(receiving TUPKP).Collected the two groups of preoperative general conditions(age,serum prostate specific antigen,prostate volume,quality of life score,international prostate symptom score),perioperative clinical data(operation time,postoperative hemoglobin reduction,gland resection weight,surgery The duration of posterior bladder irrigation,postoperative indwelling urinary catheter days,postoperative hospitalization days),postoperative complications(urinary incontinence,bladder spasm pain,postoperative gross hematuria,early postoperative dysuria),3 postoperative Monthly follow-up data(IPSS,QOL 3 months after operation).The collected data will be statistically analyzed to evaluate the clinical effects of the two surgical treatments for benign prostatic hyperplasia with a volume greater than 80 m L.Results: There was no statistically significant difference between the two groups of patients in the preoperative general condition data(P>0.05).Perioperative data of the TUKEP group: operation time,postoperative hemoglobin reduction,postoperative bladder irrigation time,postoperative indwelling catheter days,and postoperative hospital stay indexes were lower than those of the TUPKP group,and the weight index of gland resection was lower than that of the TUPKP group Higher,the difference is statistically significant(P<0.05);the postoperative complications of the two groups of patients include: urinary incontinence,bladder spasm pain,postoperative gross hematuria,and the incidence of early postoperative dysuria were not statistically significant.(P>0.05);The total complication rate in the TUKEP group was lower than that in the TUPKP group,and the difference was statistically significant(P<0.05).The IPSS and QOL scores of the TUKEP group and the TUPKP group were better than those before the operation at 3 months after the operation(P<0.05),and the difference between the groups was not statistically significant(P>0.05),indicating that the two surgical procedures were effective.Conclusions: For patients with BPH whose volume is greater than 80 ml,TUKEP has shorter operation time,more complete tissue removal,less intraoperative bleeding,shorter bladder irrigation time,shorter indwelling catheter time,faster postoperative recovery and lower overall complication rate than TUPKP.It is worthy of clinical promotion and application.
