Pathological Characteristics And Prognosis Of 147 Cases Of Ovarian High-grade Serous Adenocarcinoma

Objective:To study the prognostic analysis and pathological characteristics of high-grade serous adenocarcinoma of the ovary,and to provide a reference for individualized clinical treatment.Methods:1.The clinical data of 147 patients with high-grade serous adenocarcinoma who first visited the Department of Gynecology,Affiliated Cancer Hospital of Xinjiang Medical University from January 2011 to December 2015 were retrospectively analyzed.2.Kaplan-Meier method was used to draw the survival curve and Log-rank test was used to retrospectively analyze the factors affecting the prognosis of patients,and Cox regression was used for univariate and multivariate analysis.The correlation between the type of recurrence,immunohistochemical results and clinical factors was analyzed byχ~2.Results:(1)Univariate analysis by Kaplan-Meier method showed that patients aged more than 50years,patients with surgical and pathological stage III-IV,patients with non-standard chemotherapy,and patients with chemotherapy resistant relapse had poor prognosis.In immunohistochemistry,the positive expression of P16、P53 and WT-1 had a worse prognosis than the negative expression.(2)Cox regression multivariate survival analysis showed that age,surgical and pathological stage,standard chemotherapy,type of recurrence and P53 were independent influencing factors for high-grade serous adenocarcinoma of ovary.(3)Chi-square test analysis showed that there were differences in the recurrence types among different immunohistochemical results,that is,the recurrence types of P53 positive patients were mostly drug resistant;The recurrence type of patients with positive P16 and WT-1 expression was mostly sensitive type.Conclusions:Age,surgical and pathological stage,standard chemotherapy,recurrence type and P53 are independent influencing factors for High grade serous adenocarcinoma of the ovary.Most of the recurrence types in immunohistochemical P16 and WT-1 positive patients are sensitive type,while most of the recurrence types in P53 positive patients are drug resistant type.
