Objective:To investigate the clinical value and correlation analysis of immunoinflammatory indexes and coagulation function indexes when diagnosis of acute deep vein thrombosis of the lower extremities,and provide theoretical basis and guidance for early clinical diagnosis and individualized treatment of deep venous thrombosis of lower extremities.Methods:A total of 128 patients were selected as the DVT group with acute deep vein thrombosis who had no apparent cause and were admitted to the Department of Vascular Surgery,The Affiliated Hospital of Yangzhou University from January 2015 to December 2020,of which 64 were males and 64 were females.Diagnostic criteria:(1)The patient’s affected limb was obviously swollen with pain and all were diagnosed as DVT of the lower limb by color Doppler ultrasonography of the blood vessels of the lower limbs;(2)Onset time was less than 14 days;(3)Patients were the first Secondary illness.In addition,100 patients who had a physical examination in our hospital within the same time frame were selected.As a control group,all the investigators have completed the clinical data information statistics and blood routine and coagulation function examinations upon admission.The clinical data and examination results were counted and analyzed,and the immune inflammatory indexes and coagulation markers of the two groups were observed in the acute lower limbs.The clinical value in the diagnosis of deep vein thrombosis,using receiver operating characteristic(ROC)curve to determine the position of the cutoff point,sensitivity,specificity and area under the curve(AUC)for the best diagnosis,according to the influencing factors of acute lower extremity DVT patient Logistic stepwise regression was used to analyze the independent risk factors of acute lower extremity DVTResults:1.After chi-square test and t-test,there was no significant difference in age,body mass index,hypertension,diabetes,smoking history between the two groups(P>0.05),and the baseline data between the two groups had comparability.2.In the acute deep vein thrombosis patient group,the neutrophil count,monocytes,average platelet volume,PLR,NLR,MPVLR,MLR,SⅡ,FIB and D-dimer were all higher than those of the control group,lymphocyte count and platelet count.It was significantly lower than that of control group,and the difference had Statistical significance(P<0.05).3.According to the analysis of the AUC area under the ROC curve,it can be concluded that the AUCs of neutrophils,monocytes,MPV,PLR,NLR,MPVLR,MLR,S II,FIB,and D-dimers for the diagnosis of DVT are 0.637,0.633,0.575,respectively.,0.712,0.810,0.826,0.846,0.738,0.652,0.904,the value of the best cut-off points are 5.235×109/L,0.465×109/L,10.75 fL,107.845,2.715,5.817fL2/109,2.096,424.848x 109/L,3.366g/L,0.605mg/L,the sensitivity is 0.344,0.406,0.266,0.688,0.625,0.750,0.844,0.656,0.383,0.867,and the specificity is 0.650,0.850,0.860,0.660,0.920,0.770,0.750,0.740,0.940,0.840.4.The results of multivariate Logistics regression analysis of the risk factors of the acute deep vein thrombosis patient group showed:increased neutrophil count,decreased lymphocyte count,increased monocyte count,increased MPV count,increased NLR count,increased MPVLR count,increased MLR count,increased S II count,elevated FIB and D-dimer count are risk factors for acute lower extremity DVT.Conclusions:Increased values of neutrophils,monocytes,MPV,NLR,MPVLR,MLR,S II,and decreased lymphocyte count are immuno-inflammatory indicators that can predict and diagnose acute DVT of the lower extremities.FIB and D-dimer can be used as diagnosis the coagulation function index of acute lower extremity DVT.
