ObjectiveAnalyzed the health economics evaluation of HIV mass screening project in three demonstration counties Guangxi between 2013~2015.Markov model was established to analyze the long term cost-utility.In order to analyzed the value of HIV mass screening in different HIV epidemic level region in Guangxi.MethodsThrough field investigation to collected the implementation effect of HIV screening measures in three demonstration counties from 2013 to 2015.Standard methods of health economics were used to evaluate the HIV mass screening measures in three demonstration counties,including cost-effectiveness analysis,cost-utility analysis and cost-benefit analysis.Markov model was used to calculate the QALY through HIV mass screening measures,and analyzed the long term cost-utility analysis.Results1.The project cost amounted to 26.2062 million yuan from 2013 to2015.A total of 1641 cases HIV positive cases were found,and the positive rate was 0.067%.2.The overall screening cost of the project was 10.630 yuan,and the cost of found one case of HIV-positive was 15970 yuan,to avoid one case of HIV new infection cost 5320 yuan.The costs of found or avoid one case were9.520 ~ 23.250 yuan and 3.170 ~ 7.750 yuan in three demonstration counties.The cost of save one DALY was 256.912 yuan,and the costs of three demonstration counties were 153.215 ~373.980 yuan.The direct cost benefit ratio of project was 1: 39.662.The costs of three demonstration counties respectively were 1: 66.478,1: 27.233 and 1: 38.794.To compared the results,the indicators of A were better than B and C.3.Markov model showed that the long term cost-effectiveness was2.06 thousand yuan/QALY,without taking into account the late management cost of HIV infection.The costs of three counties were respectively between1120 ~ 3620 yuan.The sensitivity analysis showed that the results of model were stable,and the long term cost-utility has decreased with the increase of anti-viral treatment ratio.Conclusion1.A one-time mass HIV screening could be found most of the HIV occult infection,and the routine testing was likely to overestimate the actual population of the actual infection.2.The project had a good value of health economics in Guangxi.The HIV screening was complying with the principle of cost effectiveness,even if the population infection rate was 0.06% of C county.3.Strengthen the antiretroviral therapy ratio was important to improve the long term cost-utility.4.In the local economic,political and other conditions allowed,a one-time mass HIV screening was considered feasible in AIDS epidemic areas,in order to discovered the hidden infection and reduced the spread of the second generation and played the biggest benefit of health resources.
