Objective: To study whether the imbalance of Th17/Treg cell in peripheral blood of patients in different periods of frozen embryo transfer(F-ET)undergoing the occurrence of recurrent implantation failure(RIF)and play an important role,to explore the expression levels of some cell-related inflammatory factors IL-6,TGF-β,IL-17 and IL-10 in RIF patients,so as to preliminarily elaboration whether Th17/Treg cells and their inflammatory factors are jointly involved in the occurrence of RIF.Methods: 1.The data comes from 178 patients who underwent F-ET and met the inclusion criteria at Yijishan Hospital of Wannan Medical College from October 1,2019 to September 1,2021,collect fresh peripheral blood on the day of intimal transformation,the day of transplantation,and 12-14 days after transplantation of patients in the F-ET.2.Flow cytometry was used to detect the ratio of Treg cells and Th17 cells in the peripheral blood in CD4+ T lymphocytes,and the expression levels of cell inflammatory factors IL-6,TGF-β,IL-17 and IL-10 were detected by ELISA.3.It is considered that the embryo has implanted according to transvaginal ultrasound seeing the gestational sac at 28-30 days after transplantation.selected 41 patients with two or more transplants failed("RIF group")and 73 patients with the 1-2 transplants pregnant("pregnancy control group").4.(1)Compare the number and ratio of Th17 cells with Treg cells at three different periods in F-ET of the RIF group and the pregnancy control group on the day of intimal transformation,the day of transplantation,and 12-14 days after transplantation;(2)Compare the RIF group with the pregnancy control group the expression changes of cell inflammatory factors IL-6,TGF-β,IL-17 and IL-10 at three different periods.(3)Analyze the correlation between Th17/Treg cells and their inflammatory factors.Results: 1.Compared with the pregnancy control group,the number of Th17 cells in peripheral blood increased on the day of intimal transformation,the day of transplantation,and 12-14 days after transplantation in the RIF group,while the number of Treg cells was the decreased(P < 0.05),and the ratio of Th17/Treg cells was significantly increased(P<0.01).2.The level of IL-6,IL-17 in the RIF group at three periods was higher than the pregnancy control group,and the level of IL-10 decreased(P < 0.05),while the content of TGF-β between the two groups had no significant difference in the three periods(P > 0.05),and the ratio of cell differentiation factor IL-6/TGF-β and cell secretion factor IL-17/IL-10 are significantly increased(P<0.05).3.The correlation analysis of the three periods of F-ET indicated that IL-6/TGF-β,IL-17/IL-10 and Th17/Treg cell ratios were significantly positively correlated(r≥0.8).Conclusion: 1.The imbalance of Th17/Treg cell ratio may be involved in the occurrence of RIF,which is mainly manifested by the number of Th17 cells increased and the number of Treg cells decreased.2.The increased ratio of IL-6/TGF-β and IL-17/IL-10 related inflammatory factors of Th17 cells and Treg cells may be involved in the occurrence of RIF,manifested by IL-6 and IL-17 levels increased while IL-10 levels decreased,which are related to Th17/Treg cell imbalance.The results of this experiment provide experimental and theoretical basis for the prediction and clinical treatment of RIF,and have important clinical significance for improving pregnancy outcomes in some RIF patients.
