| Objectives Investigate and analyze the influential factors of senile heart failure patients frailty and observe the effect of nutrition intervention on senile heart failure patients frailty.Methods 1 Study on the influencing factors of frailty in elderly patients with heart failure: Patients with heart failure diagnosed as frailty who were admitted to the Department of Geriatrics and General Medicine of the Affiliated Hospital of North China University of Science and Technology from September 2019 to May 2020 were successively selected as the case group(frailty group);Patients diagnosed with heart failure but without frailty were selected as the control group(non-frailty group).According to the questionnaire and evaluation form,the general data,laboratory data and echocardiography data of the patients were collected.The collected data were recorded into a computer to establish a database,and the collected data were analyzed by SPSS17.0software.In univariate analysis,the measurement data conforming to the normal distribution were statistically described as "mean ± standard deviation",and the mean between groups was compared using the independent sample t test.The chi-square test was used to compare the composition of counting data groups.Logistic regression model was used for multivariate analysis.Two-sided test,P<0.05 was considered statistically significant.2 Nutritional intervention study: Patients diagnosed with heart failure in the geriatrics department and general practice department of the Affiliated Hospital of North China University of Science and Technology from September 2019 to May 2020 were selected continuously,and Fried fission assessment was conducted,and the assessment results were in the fission stage.They were divided into intervention group and control group.General data and frailty related indexes of patients were collected.Nutritional intervention was performed,and frailty-related indicators were evaluated at the beginning of the intervention and 3 months later to evaluate the effectiveness of the intervention.Results 1 Influencing factors of frailty in elderly patients with heart failure:(1)Univariate analysis showed that:(1)There were no significant differences in gender,smoking history,drinking history and ADL scores between frailty group and non-frailty group(P>0.05);Age,number of multiple drug use,number of nutritional risk,length of hospital stay and walking time in frailty group were higher than those in non-frailty group,while body mass index(BMI)and grip strength were lower than those in non-frailty group,with statistical significance(P<0.05).(2)There was a statistically significant difference in the age composition between the frailty group and the non-frailty group(P<0.05).The proportion of advanced age in the frailty group was higher than that in the control group(P<0.05).(3)There was no statistically significant difference in the proportion of patients with coronary heart disease,hypertension and thyroid dysfunction between the frailty group and the non-frailty group(P>0.05),while the proportion of patients with cerebrovascular disease,diabetes and chronic obstructive pulmonary disease in the frailty group was significantly higher than that in the non-frailty group,with statistical significance(P<0.05).(4)There were no significant differences in the levels of white blood cell(WBC),total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)between the frailty group and the non-frailty group(P>0.05).The levels of hemoglobin(HGB),albumin(ALB)and high-density lipoprotein cholesterol(HDL-C)in the frailty group were significantly lower than those in the non-frailty group,and the levels of serum creatinine(CRE),uric acid(UA)and amino terminal brain natriuretic peptide precursor(NT-pro BNP)were higher than those in the non-frailty group.There were statistically significant differences among each index group(P<0.05).(5)There was no significant difference in E/A ratio between the frailty group and the non-frailty group(P>0.05);The left atrial diameter(LAD)level of frailty group was higher than that of non-frailty group,and the left ventricular ejection fraction(LVEF)level of frailty group was significantly lower than that of non-frailty group,with statistical significance(P<0.05).(2)Multivariate analysis showed that: Age,multiple drug use,nutritional risk,HGB,NTpro BNP,LAD,LVEF and the occurrence of fission are independently related(P<0.05),advanced age,multiple drug use,nutritional risk,increased NT-pro BNP,LAD increase are the risk factors of fission in elderly patients with heart failure,High HGB and high LVEF are protective factors for frailty in elderly patients with heart failure.2 Nutrition intervention study: Before intervention,there were no significant differences in age,gender,body mass index,smoking history,drinking history,flimsiness,grip strength and walking time between the intervention group and the control group(P<0.05).The frailty of both groups was improved after intervention,and the intervention group was better than the control group,the difference was statistically significant(P<0.05).After intervention,the grip strength of the intervention group was improved,but the difference between the two groups was not statistically significant(P<0.05).After intervention,walking time in both groups improved,and the intervention group was lower than the control group,the difference was statistically significant(P<0.05).Conclusions 1 Age,multiple drug use,nutritional risk,HGB,NT-pro BNP,LAD,LVEF were associated with fission in elderly patients with heart failure.Elderly age,multiple drug use,nutritional risk,increased NT-pro BNP,LAD,and increased risk of fission;HGB increased and LVEF increased,and the risk of frailty decreased.2 Nutrition intervention can improve the walking time of
