| Objectives To explore the risk factors of kinesiophobia in patients after total knee arthroplasty,and provide a basis for clinical development of early intervention measures for kinesiophobia.Methods From October 2019 to September 2020,the patients who underwent initial unilateral total knee arthroplasty in the orthopedic ward of the First Affiliated Hospital of Zhengzhou University were selected as the research subjects.First of all,on the 5th day after the patient’s postoperative condition were stable,the same version of the Tampa Scale for Kinesiophobia(TSK)was used to select the study subjects of the case group and the control group.Then,170 cases of panic disorder-positive patients with a TSK score>37 were included in the case group,and 340 hospitalized panic disorder-negative patients with a TSK score of≤37 were included in the control group.Subsequently,a self-designed risk factor questionnaire was used,which included the patients’general demographic data,and the patients’daily health behavior at home,and the patients’disease-associated condition,and the patients’psychological condition,and the patients’personal coping style,and the medical service related factors,and the hospital ward environment factors,and the patients’family support status,and then compared and analyzed the distribution differences between the case group and the control group in the above aspects.When sorting and analyzing the data,all data were entered using EXCEL,and data analysis was performed using SPSS25.0.In this process,a descriptive analysis of general demographic data was performed,a X~2 test was used for univariate analysis,and a binary Logistic regression was used for multivariate analysis.Results Logistic regression analysis of risk factors for kinesiophobia in patients after total knee replacement:The results show that:the patients’Family Care Index were poor(OR=6.949),and the medical staff occasionally forced the patients to exercise when the patients were unwilling(OR=5.988),and the patients’family members’recovery confidence were low(OR=3.974),and the patients’postoperative self-efficacy were low(OR=3.659),and the exercise pain of other patients’would greatly affect the patients(OR=2.756),and the medical staff often forced the patients to exercise when the patients were unwilling(OR=2.517),and the patients’postoperative anesthetic side effects were relatively serious(OR=2.404),and the higher avoidance tendency in postoperative medical coping styles(OR=2.134),and the patients’family members’recovery confidence were medium(OR=2.095),and the patient were already depressed before surgery(OR=2.003),and the higher yield tendency in postoperative medical coping styles(OR=1.709),were the risk factors for kinesiophobia in the patients after total knee replacement(P<0.05).Conclusions The risk factors for kinesiophobia in patients after total knee replacement were:the patients’Family Care Index were poor,and the medical staff frequently forced the patients to exercise when the patients were unwilling,and the patients’family members’recovery confidence were low,and the patients’postoperative self-efficacy were low,and the exercise pain of other patients’would greatly affect the patients,and the patients’postoperative anesthetic side effects were relatively serious,and the high avoidance tendency in postoperative medical coping styles,and the patients were already depressed before surgery,and the high yield tendency in postoperative medical coping styles.The worse the patients’Family Care Index,and the higher the frequency of exercise supervised by the medical staff when the patients were unwilling,and the lower the recovery confidence of the patients’family members’,and the lower the patients’postoperative self-efficacy level,and the exercise pain of other patients’would much more greatly affect the patients,and the more severe the patients’postoperative adverse reactions of anesthetics,and the higher the patients’avoidance tendency of the medical coping style,and the more severe the depression that the patients already existed before the operation,and the higher the patients’yield tendency of the medical coping style,the greater the risk of kinesiophobia in patients undergoing total knee arthroplasty.Figure1;Table13;Reference143... |
