Objectives : To explore the effectivity and safety by applying rt-PA intravenous thrombolysis to acute ischemic stroke patients aged 80 years and over.Methods:198 acute ischemic stroke patients were retrospectively collected and divided into three groups according their age and by applying intravenous Thrombolysis or not:elderly thrombolysis group including 46 patients treated with Rt-PA intravenous thrombolysis aged 80 years or older,non-thrombolytic group with 62 cases who were not treated with rt-PA intravenous thrombolysis and were over 80 years old,and younger thrombolysis group with 90 patients treated with rt-PA intravenous thrombolysis aged less than 80 years.Evaluate the effectiveness by comparing the NIHSS scores at 7 days and the modified Rankin Scale(m RS)at 90 days after treatment. The safety was also assessed by intracranial hemorrhage, symptomatic intracranial hemorrhage and mortality within 90 days. Results: The elderly thrombolytic group, the NIHSS scores at 7 days and good prognosis rate at 90 days was significantly lower than that of the elderly non thrombolytic group, but it is higher than that of the younger thrombolysis group. The conversion rate of intracranial hemorrhage in the elderly thrombolytic group was significantly higher than that of the elderly non thrombolytic group(P<0.05),while no significant difference in the incidence of symptomatic intracranial hemorrhage and the incidence of gastrointestinal hemorrhage between the elderly thrombolytic group and the younger thrombolytic group(P>0.05).Indeed,no significant difference in the conversion rate of intracranial hemorrhage, the incidence of symptomatic intracranial hemorrhage and the incidence of gastrointestinal hemorrhage and in terms of 90 day mortality between the elderly thrombolytic group and the younger thrombolytic group(P>0.05).Conclusions: Application of rt-PA intravenous thrombolysis to acute ischemic stroke patients can significantly improve the curative effect and prognosis.
