A Retrospective Study Of The Efficacy Of Yiqi Xiaozheng Decoction Combined With Chemotherapy Treatment Patients With Advanced Non-small Cell Lung Cancer

1.ObjectiveThis paper is mainly to retrospectively analyze clinical effect and the impact on TCM syndrome score,tumor-associated antigens,survival time of Yiqi Xiaozheng decoction combined with chemotherapy treatment patients with stage IIIB-IVB non-small cell lung cancer(NSCLC)based on the datebase of the hospital clinical medical records system,and the risk factors influencing the clinical curative effect and prognosis of survival will be explored either,then discussing the basic theory of TCM and western medicine pharmacological mechanism of Yiqi Xiaozheng decoction tremting lung cancer,to provide ideas for TCM treatment of NSCLC,and to lay the foundation for exploring specific effective prescription for the treatment of NSCLC.2.MethodBy our hospital clinical medical records system,screening on January 1,2013 to December 31,2019,accords with the standard set of cases,observing the clinical effect,TCM syndrome score,tumor-associated antigens,and follow-up survival time of Yiqi Xiaozheng decoction combined with chemotherapy treatment of advanced NSCLC,with the help of SPSS21.1 statistical software to analyze data,and stratified according to different standards,to analyze the related factors influencing the clinical efect rate and survival prognosis.3.Results(1)In terms of curative effect of solid tumor,objective remission rate and disease control rate were 17.07% and 86.58% after 2 courses of treatment;The objective remission rate and disease control rate were 23.08% and 76.92% after 4 courses of treatment.The objective remission rate was 39.06% and the disease control rate was 79.69%.The factors that had statistical significance on the short-term clinical benefit rate included the card score(P=0.022),the degree of tissue differentiation(P=0.038),the number of abnormal tumor markers(P=0.023)and the course of disease(P=0.005).(2)Compared with before treatment,TCM syndrome scores decreased significantly after treatment,and the difference was statistically significant(P=0.000),and there was statistically significant difference in TCM syndrome scores between different courses of treatment(P=0.000).(3)The decrease degree of CEA,CA125,CA153 and CA199 after treatment was statistically significant(P=0.000).(4)In terms of survival time,the median PFS was 6.5±0.418 months,and the median OS was 12.5 ± 0.81 months.The direct factors affecting the overall survival time were the degree of tissue differentiation(P=0.000),the card score(P=0.000)and the number of tumor-associated antigen abnormalities(P=0.000),and the pleural effusion indirectly affects OS(P=0.01).(5)In terms of toxic reactions,the incidence of myelosuppression was 38%,including grade I 20 cases,grade II 6 cases,grade III 3 cases,and grade IV 2 cases;The incidence of nausea and vomiting was 13.41%.The incidence of liver function impairment was13.41%.The incidence of impaired renal function was 8.5%.4.ConclusionCompared with western medicine chemotherapy alone,although Yiqi Xiaozheng decoction combined with chemotherapy did not significantly improve DCR,nor significantly prolong the survival of patients,but it had relatively high DCR and longer median PFS and median OS,which could reduce the incidence of toxic and side effects After treatment in patients with TCM syndrome integral,tumor-associated antigens were significantly decreased,and longer duration of taking Chinese traditional medicine,TCM syndrome scores remain declined,and KPS,different differentiation,numbers of abnormal tumor-associated antigens and course of disease are factors that influence the DCR,KPS is higher,the differentiation of organization for the high,the abnormal tumor-associated antigens number is less,and treatment is starting earlier,the short-term effect and prognosis is better.The factors that infect OS include KPS,the differentiation of organization,and the number of abnormal tumor-associated antigens,whether the merger pleural effusion is the indirect factor that affects OS.Age,gender,pathological type,TNM staging,TCM syndrome type had no significant effect on the DCR and OS.From the perspective of traditional Chinese medicine theory,Yiqi Xiaozheng decoction fits the etiology and pathogenesis of lung cancer perfectly,so it can be suitable for different TCM syndromes,and is an effective specific prescription for the treatment of lung cancer.
