ObjectiveTo analyze and summarize the correlation between Traditional Chinese Medicine(TCM)syndrome patterns in patients with colorectal polyps(CRP)and Helicobacter pylori(Hp)infection and other factors,so as to understand the risk factors of CRP and the distribution of TCM syndrome types,and provide theoretical basis for TCM Prevention and treatment of CRP in Lingnan area.MethodsThe general information,TCM syndrome patterns,13 C urea breath test,colonoscopy and other auxiliary examination data of patients with CRP hospitalized in the First Affiliated Hospital of Guangzhou University of Chinese medicine from January 2020 to December2020 were collected and collated.Excel was used to collect and manage the data,and the correlation between TCM Syndrome patterns and pathological types,endoscopic characteristics,Hp infection and other factors of patients with CRP were statistically analyzed with SPSS 25.0.ResultsTotally 295 cases were included,the maximum age was 76 years old and the youngest age was 19 years old,the median age was 55 years old,and the ratio of male to female was3.2:1.Most of them were middle-aged and elderly patients(patients over 40 years old accounted for 84.75%).There were 151 cases of Hp infection(51.19%).Patients with multiple polyps are predominant(76.61%).There were significant differences in the number of polyps among gender,age,blood lipid and Hp infection(P < 0.05).Men,middle-aged and elderly people,dyslipidemia and Hp infection patients were more likely to have multiple polyps.There was no correlation between body mass index(BMI)and the number of polyps(P > 0.05).The diameter of CRP ranged from 2 to 30 mm,and the median diameter was 8mm.The number of patients with a diameter of 5~10 mm was the largest(55.59%),and≥20mm was the least(6.10%).There were significant differences in the size distribution of polyps between BMI and Hp infection(P < 0.05).BMI abnormality was more likely in ≥ 20 mm group,and Hp infection was more likely in ≥ 10 mm group.There was no significant difference in the size distribution of polyps among genders,ages and blood lipids(P > 0.05).Sigmoid colon was the most common site of CRP(33.44%),and cecum was the least(3.85%).There was no significant difference in the distribution of polyps in different gender,age,blood lipid,BMI and Hp infection(P>0.05).The frequency of pathological types was 598 without villous adenoma.The frequency of adenomatous polyps was 347(58.03%)and non-adenomatous polyps was 251(41.97%).Tubular adenoma was the most common pathological type(44.48%),followed by inflammatory polyps(33.44%).There were significant differences in the distribution of pathological type among different BMI,lesion location,polyp size and Hp infection(P<0.05).Most of the inflammatory polyp and tubular adenoma were the pathological types in patients with Abnormal BMI.The most common pathological type in cecum,ascending colon,transverse colon,descending colon and sigmoid colon were tubular adenomas.The most common pathological type in patients with diameter less than 5mm was inflammatory polyp,diameter 5 ~19mm was tubular adenoma,and diameter no less than 20 mm was tubular villous adenomas.Hp infected patients had a predominance of tubular adenomas.There was no significant difference in the distribution of pathological types among different genders,ages and blood lipids(P>0.05).In TCM syndrome patterns,187 cases(63.39%)were spleen deficiency and dampness stasis syndrome,102 cases(34.58%)were intestinal damp heat syndrome,and 6 cases(2.03%)were qi stagnation and blood stasis syndrome.The number of patients with the pattern of spleen deficiency and dampness stasis syndrome was the most.There was significant difference in the distribution of syndrome pattern among different genders,blood lipid and Hp infection(P<0.05).Spleen deficiency and dampness stasis syndrome was the main syndrome in both sexes,and the proportion of female was higher than that of male(74.29%for female and 60.00% for male).The Hp positive rate and dyslipidemia rate of spleen deficiency and dampness stasis syndrome was higher.There was no significant difference in the distribution of syndrome patterns among different polyp size,location,pathological type,age and BMI(P>0.05).ConclusionThere are gender and age differences in CRP.Male and middle-aged and elderly patients were prone to develop polyps.Patients with dyslipidemia had more polyps.Patients with Abnormal BMI had larger polyps.Patients with Hp infection had more polyps with larger polyps.It suggested that gender,age,blood lipid,BMI and Hp infection might be risk factors for CRP.Combined with the literature,the correlation between Hp infection and CRP still needed further study.Sigmoid colon was the most common site of CRP,and the main pathological type of CRP was tubular adenoma.The distribution of pathological types was different in different parts.The main pathological type of rectum was inflammatory polyp,and the other parts were tubular adenoma.The main TCM syndrome pattern was spleen deficiency and dampness stasis syndrome.Hp infection and blood lipid affected the distribution of syndrome patterns,and the HP positive rate and dyslipidemia rate of spleen deficiency and dampness stasis syndrome was higher.
