| Objective:To draw Lorenz scatterplot of simple premature beat and parallel rhythm,study the distribution of each scatterplot and summarize its characteristics.Objective to analyze whether we can quickly and accurately identify simple premature beat and parallel rhythm according to the distribution of characteristic Lorenz scatterplot,and compare with the traditional ECG diagnostic method,to study the advantages of Lorenz scatterplot in differential diagnosis,and to reflect the clinical application value of Lorenz scatterplot.Methods:Patients with frequent premature contractions in our hospital from 2016 to 2019 were selected and divided into two groups according to the diagnosis results of 12 channel dynamic electrocardiogram(DCG).Group I(simple premature contractions)and group II(parallel rhythm)were counted.Lorenz scatter diagram was drawn according to the data recorded by DCG.According to the point set distribution characteristics of Lorenz scatterplot,it is classified and counted.The diagnostic results of Holter and Lorenz scatter plot analysis were compared,and the reliability of ECG scatter plot analysis was studied.The differences of diagnostic criteria between 24-hour Holter and ECG scatter plot analysis were compared.Find out the medical records with the characteristics of Lorenz scatter plot model of parallel rhythm,and divide them into atrial parallel rhythm,junctional parallel rhythm,ventricular parallel rhythm,double parallel rhythm and pseudo parallel rhythm according to the body surface routine ECG.Analyze the relationship with their clinical background,and compare the differences between ventricular parallel rhythm and simple ventricular premature beat in cardiovascular disease,Objective to clarify the clinical significance of parallel rhythm.Results:1.In the differential diagnosis of simple premature beat and parallel rhythm,the coincidence rate of supraventricular premature beat,ventricular premature beat and parallel rhythm was 99.66%,99.95% and 89.43%,respectively,The diagnosis is reliable.2.On the scatter plot,the differences between the distribution of each point set of simple premature contraction and parallel rhythm were compared.The angle of long axis of premature contraction point was significantly lower than that of parallel rhythm group,while the angle of long axis of premature contraction point was significantly higher than that of parallel rhythm group(P < 0.05),It can be seen that the scatter plot analysis method of ECG has great differences among the distribution figures of each point set,and the figure is intuitive.It is easier to identify simple premature contraction and parallel rhythm.3.When analyzing 187 cases of ventricular parallel rhythm,the analysis method of ECG scatter plot is different from that of 24-hour dynamic ECG template cluster analysis in detecting fusion wave,short syndromic interval(premature beat index 〈 1)and fast frequency dependent efferent block(dominant slow ventricular rate).In the analysis of 27 cases of borderline parallel heart rhythm,the two analysis methods were different in revealing the excursion of excitation points,revealing the short ventricular refractory period(premature beat index 〈 1)and revealing the second degree efferent block.The chi square test showed that the difference was statistically significant(P < 0.05),There are obvious advantages in capturing event details,It has important clinical value to provide a strong basis for clinical clearer and more precise judgment and reduce the missed diagnosis of parallel rhythm.4.Compared with the clinical background of heart disease,the parallel rhythm group was higher than the simple premature contraction group,P = 0.013(P 〈 0.05),the difference was statistically significant,and the differential diagnosis of parallel rhythm has important clinical significance.Conclusion:Lorenz scatter plot analysis method is used to differentiate simple premature beat and parallel rhythm.The diagnosis result is reliable,and the detail processing is superior to the traditional ECG analysis method. |
