Objective: To understand the prevalence and genotype distribution of cervical high-risk human papillomavirus(hr HPV)in Fushun area,to study the related factors of hr HPV infection,and to compare the efficacy of colposcopy referral strategy in hr HPV-positive population.Discuss the difference and influencing factors of cytological diagnosis results between the preliminary screening group and the quality control group.As a sub-center of a large multi-center clinical randomized controlled project,it also provides reference and basis for China’s cervical cancer screening strategy and cervical cancer vaccine development and promotion strategy.Methods: A cross-sectional study was conducted using a hospital in Fushun City as a cervical cancer screening site.Cervical specimens were taken from women aged 35 to 65 years.The samples were examined by cytology and hr HPV.Any abnormal results were referred to colposcopy,and pathological tissue biopsy was performed based on cytology and colposcopy.For cytological abnormalities and negative smears,10% were randomly selected for quality control in higher-level hospitals.Use SPSS.24 to analyze the difference of statistics and related factors for screening data.Results: A total of 3000 women were recruited and 2357 women provided data for this study.The positive rate of hr HPV was 19.8%,and the incidence of CIN2 and above was only 0.64%.Age,menopause,and contraceptive measures all affect hr HPV infection.The top five hr HPV genotypes in Fushun area are HPV16,52,58,66,56 respectively.In hr HPV-positive patients,the test efficiency of the shunt strategy using genotype combined with cytology is generally better than that of a single method.In addition,the TCT preliminary screening results of multiple infections at the same time,especially when infected with hr HPV after menopause,are more likely to be inconsistent in quality control.Conclusion: Taking into account the distribution of hr HPV in Fushun area,the HPV vaccine currently inoculated with bivalent and quadrivalent vaccines in Fushun may be replaced by nine-valent vaccines in the future.In the hr HPV-positive population,the shunting strategy of hr HPV genotype combined with cytology is generally better than using a single method for shunting regardless of sensitivity,specificity,positive prediction rate,and negative prediction rate,and should be selected according to clinical needs.In addition,the consistency of the cytological smear review results of the primary screening in the primary hospitals is low.The factors that affect the consistency of the diagnosis of the scan include hr HPV infection and menopause.Therefore,the number and quality of pathologists In weaker areas,quality control with higher-level hospitals should be strengthened.
