| Objective: To analyze and compare the effects of laparoscopic total mesorectal resection(laTME)and transanal total mesenteric resection(taTME)on the prognosis of patients with low rectal cancer;evaluate the application value of transanal resection of total mesentery;study different risk factors Impact on the prognosis of patients with low rectal cancer.Methods: A retrospective analysis of the clinical data of 73 patients who underwent TaTME in the Department of Colorectal Tumor of Shengjing Hospital Affiliated to China Medical University from September 2016 to September 2020,matched the same period of laTME surgery patients with the same characteristics,and compared the two Group intraoperative,postoperative and prognostic indicators.Results: The average operation time of taTME was longer than that of laTME group,the proportion of ileal protective stoma was higher than that of laTME group,the proportion of manual intestinal anastomosis was higher than that of laTME group,the average blood loss during operation was more than that of laTME group,and the total postoperative complication rate was higher.It is lower than the laTME group and the difference is statistically significant.However,the maximum diameter of the tumor,the distance from the distal resection margin to the anus,the positive rate of the distal resection margin and the circumferential resection margin,the number of lymph nodes obtained,the integrity of the rectum,and the postoperative TN staging There was no statistical difference between the two groups in terms of the proportion of conversion to open surgery,the number of days of hospitalization after surgery,postoperative anal function and short-term prognosis.Conclusion: taTME is comparable to laTME in terms of surgical,oncology,anal function,prognosis results,but has a lower incidence of postoperative complications. |
