Objective: About 30% of the patients with epilepsy are suffering from drug-resistant epilepsy,and the potential drug-resistant mechanisms remain unclear.Minimally invasive,reliable and affordable biomarkers are helpful to early diagnosis and judge the prognosis of drug-resistant epilepsy.We aimed to explore the role of miRNA-200c-3p and miRNA-455-3p in the drug-resistant epilepsy.Methods: 90 patients with epilepsy admitted to our hospital and 36 healthy people were enrolled in our study.They were divided into drug-resistant epilepsy group,drugresponsible epilepsy group and healthy controls.Baseline clinical data of all the 126 people were collected,and assessment of cognitive level and psychological status were performed for everyone.Fluorescence quantitative polymerase chain reaction technology was used to test the expression level of miRNA-200c-3p and miRNA-455-3p in the peripheral blood of all participants.One-way anova was used to the assess the expression level of miRNA-200c-3p and miRNA-455-3p between three groups.Receiver operating curve(ROC)was created to assess the diagnostic value of miRNA-200c-3p,miRNA-455-3p and the combination of the two to drug-resistant epilepsy.Results: There were 39 patients with drug-resistant epilepsy and 51 patients with drugresponsible epilepsy.Our results show that patients with drug-resistant epilepsy are more vulnerable to psychological diseases(P=0.003)and cognitive disorders(P=0.004).Additionally,temporal lobe epilepsy(P=0.024),epilepsy family history(P=0.037)and brain MRI abnormalities(P=0.044)are more frequently observed in drug-resistant epilepsy group.Regarding the expression level of miRNA-200c-3p and miRNA-455-3p,drug-resistant epilepsy group>drug-responsible epilepsy group>healthy controls(P<0.001).The area under the curves of the ROC for miRNA-200c-3p,miRNA-455-3p and the combination of the two to diagnosing drug-resistant epilepsy were 75.1%(P<0.001),82.8%(P<0.001)and 93.8%(P<0.001),respectively.The results show that the combination of two is more valuable for diagnosing drug-resistant epilepsy,compared with the either one solely.Conclusion: Patients with drug-resistant epilepsy are more vulnerable to psychological diseases and cognitive disorders.Additionally,temporal lobe epilepsy,epilepsy family history and brain MRI abnormalities are more frequently observed in patients with drug-resistant epilepsy.The combination of miRNA-200c-3p and miRNA-455-3p is valuable for diagnosing drug-resistant epilepsy.The mentioned above maybe considered in the diagnosis and adjust of therapeutic regimen for patients with epilepsy.
