| Objective:To investigate the risk factors of lateral lymph node metastasis in patients with locally advanced rectal cancer after neoadjuvant chemotherapy.Methods:A retrospective analysis of clinical case data of middle and low rectal cancer patients with preoperative neoadjuvant chemotherapy+lateral lymph node dissection in Shengjing Hospital of China Medical University from September 2018 to December2020 in General Surgery,and SPSS software version 24.0 was used Statistical analysis was performed,measurement data was expressed as X±S,and independent sample t test was used for comparison between groups;pairwise correlation analysis of enumeration data was performed by continuous correction X~2test or X~2test;single factor analysis of meaningful indicators was performed again for binary variables Logistic regression analysis;P≤0.05 indicates that the difference is statistically significant.Results:A total of 43 cases of clinical data that met the criteria were included in the study.The univariate results suggested:irregular edges of lateral lymph nodes,uneven density of lateral lymph nodes,postoperative pathology with regional lymph node metastasis,and short axis diameter of lateral lymph nodes after neoadjuvant chemotherapy>7mm Related to lateral lymph node metastasis(P≤0.05);logistic regression analysis of the above related factors showed that postoperative pathology had regional lymph node metastasis,and the short axis diameter of lateral lymph node>7mm after neoadjuvant chemotherapy was an independent risk of lateral metastasis of rectal cancer Factors(P≤0.05).Conclusion:After neoadjuvant chemotherapy,lateral lymph nodes have irregular edges,uneven density,short axis diameter>7mm,and regional lymph node metastasis are risk factors for lateral lymph node metastasis of rectal cancer.For patients with clear indications of lateral lymph node metastasis after neoadjuvant chemotherapy,complete mesorectal resection+lateral lymph node dissection can achieve better treatment results. |
