| Objective: To compare the clinical efficacy between hemoperfusion and gamma globulin of Henoch Schonlein purpura with gastrointestinal bleeding in children.Methods: A total of 39 children with Henoch Schonlein purpura and gastrointestinal bleeding diagnosed by the Department of pediatric rheumatology and immunology in our hospital from January 2015 to December 2019 were included.According to whether the children treated by hemoperfusion or gamma globulin,they were divided into hemoperfusion group and gamma globulin group.The clinical data were collected,and the children included were followed up for 6 months to understand whether there was recurrence of gastrointestinal bleeding and renal injury,so as to analyze advantages and disadvantages of two therapeutic schemes.Results: 1)There were 20 cases in hemoperfusion group(2 cases was treated by gamma globulin before hemoperfusion)and 19 cases in the gamma globulin group.There was only age difference between the two groups,but there was no difference in gender and days of onset;2)In addition to gastrointestinal bleeding,the two groups also had other clinical symptoms,such as abdominal pain,angioneurotic edema,gross hematuria,etc.All cases were mixed purpura;3)There was no significant difference between the two groups by abdominal imaging examination and some laboratory indexes such as WBC,FDP,DD and albumin at all;4)After HP / gammaglobulin treatment,the clinical symptoms and laboratory indexes of the two groups were significantly improved,but there was no significant difference between the two groups.HP / gamma globulin treatment within 3 days could significantly shorten the length of hospital stay and reduce the cost of hospitalization;5)There was no significant difference in the dosage of hormone and the safety between the two groups;6)The results of discharge in the perfusion group were better than those in gamma globulin group.One child in gamma globulin group still had gastrointestinal bleeding when left the hospital,and there was no significant difference in the prognosis between the two groups.Conclusion: For children with Henoch Schonlein purpura and gastrointestinal bleeding,HP treatment within 3 days can quickly relieve clinical symptoms,reduce hospitalization days and hospitalization costs.If there is no hemoperfusion condition,gamma globulin is also an effective treatment. |
