| Objective: To compare the effect of metformin and simple diet and exercise therapy in children with non-alcoholic fatty liver disease(NAFLD)and assess the therapeutic value of metformin in children with NAFLD.Method: 61 cases of children who were diagnosed as NAFLD in Shengjing Hospital Affiliated To China Medical University pediatric digestive department between May2017 and September 2020 were included into this study.All patients is randomly divided into simple lifestyle intervention group(26 cases)and metformin group(35 cases).Patients in the simple lifestyle intervention group were established diet and exercise therapy.Patients in metformin group took metformin on the basis of improving lifestyle.Physique index,liver enzyme level,blood lipid level and insulin resistance index were compared before and after 6 months.Results: Compared with before treatment,body mass index(BMI)and waist-to-height ratio were significantly reduced,and no statistical significance(P>0.05)between 2 groups.Fasting serum insulin,insulin resistance index decreased after treatment in metformin group(P<0.05),no significant difference in simple lifestyle intervention group(P>0.05).The decrease of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and gamma glutamyl transferase(GGT)level has significantly difference in all patients compared with before treatment,and no statistical significance(P>0.05)between 2groups.The both groups had no significant improvements in triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)(P>0.05).Conclusion: Simple lifestyle intervention can reduce weight and the transaminase level in children with NAFLD,but has no obvious improvement in sugar metabolic disorders.Metformin in combination with lifestyle intervention can not only reduce weight and liver damage,but also improve the sugar metabolism disorder in children with NAFLD. |
