| Objective: Varicocele is one of the most common diseases in urology.In this study,patients with primary varicocele were treated with three different operations.The time of operation,the improvement of early symptoms and recurrence after operation,and the improvement of semen quality before and after operation were retrospectively analyzed,and the curative effects of three different surgical methods were compared and analyzed to lay a good foundation for follow-up surgical treatment of the disease.Methods:A total of 82 patients,aged from 19 to 50 years old,with an average of 27.63±4.32 years old,were selected for surgical treatment in our hospital from 2018 to 2020.The 82 patients were divided into three groups: laparoscopic cluster group(n = 27),high selection group(n = 27)and microscope group(n = 28).All patients in each group had primary left varicocele.The three groups of patients were treated with laparoscopic varicocele ligation,laparoscopic highly selective spermatic vein ligation and microscopic spermatic vein ligation,and the relevant data of each group before and after operation were collected,including preoperative general information(age,BMI index,etc.),semen routine results before and after operation,operation time(including anesthesia time),postoperative follow-up data(postoperative recurrence rate,complications,remission of scrotal discomfort,etc.),and these data were compared and analyzed.The method has been approved by the Ethics Committee and informed consent.Results: There was no significant difference in preoperative general data among the three groups(P > 0.05).The operation time of the three groups was analyzed and compared: the operation time of the cluster group was significantly lower than that of the microscope group and the high choice group(P < 0.05).The the number of(a + b)grade motile sperm and total motility and sperm concentration in the three groups were analyzed and compared:the number of(a + b)grade motile sperm in all patients after operation was higher than that before operation,and the number of(a + b)grade motile sperm in microscope group and high selection group was higher than that in cluster group(P < 0.05).The sperm concentration of all patients after operation was also higher than that before operation,and the rising degree of sperm concentration in microscope group and high selection group was better than that in cluster group(P < 0.05).The postoperative follow-up data of the three groups were analyzed and compared: there was no significant difference in postoperative recurrence rate,testicular hydrocele rate,postoperative complications and symptom relief among the three groups(P > 0.05).Conclusions: In the treatment of primary varicocele,the above three operations all have good results.Compared with the other two groups,the ligation operation in the cluster group has the advantage of the shortest operation time,but after the surgical treatment,the improvement of sperm quality is the worst;the other two groups have their own advantages and disadvantages.From a clinical point of view,these two kinds of operations are effective ways for the treatment of varicocele,and the most ideal operation can be selected according to the actual situation. |
