Objective: Using Optical Coherence Tomography Angiography(OCTA)to get the characteristics of the choriocapillary flow in hypertensive patients.Aim to analyze the relationship between them and systemic factors. Methods: This study is a retrospective study. The observation objects of this study were92 hypertensive patients who were treated in the Hypertension Center of the first hospital of China medical university from September 2018 to December 2019.All patients underwent intraocular pressure, fundus photography, and OCT-A examination to get their choriocapillary layer. The image of the capillary vascular layer is processed by Image J software in order to get the characteristics of the choriocapillary flows. Then,we will explore the relationship between the characteristics and systemic factors of human body. Results: In the univariate analysis, patients with smoking and decreased cerebral artery compliance had more flow voids. In simple linear regression,age(β=0.36,95%CI [0.159-0.551],p=0.0005),ascending aortic inner-diameter(β=1.06,95%CI[0.316-1.794],p=0.0056),high-density lipoprotein cholesterol(β=14.22,95%CI[6.00-22.44],p=0.0009)and the area of flow voids all have linear relationships with the area of flow voids, and they are all positively correlated.Age(β=8.85,95%CI[4.05-13.64],p=0.0004),highdensity lipoprotein cholesterol HDL-C(β=447.20,P<0.0001),common carotid artery intima-media thickness IMT(β=586.30,95%CI[83.36-1089],p=0.0232),Cystatin C(β=176.30,95%CI[5.458-347.1],p=0.0429)and serum uric acid(β=-0.811,95%CI[-1.529--0.093],p=0.0271)all have linear relationships with the size of flow voids,age,HDL-C,Both IMT and Cystatin C are positively correlated, while serum uric acid is negatively correlated.Age(β=-94.71,95%CI[-138.3--51.14],p<0.0001),ascending aortic innerdiameter(β=-276.6,95%CI[-444.3--108.9],p=0.0015),high-density lipoprotein cholesterol HDL-C(β=-3236,95%CI[-5129--1344],p=0.0010),serum uric acid(β=7.475,95%CI[0.560-14.39],p=0.0342)all have linear relationships with the number of flow voids, age, ascending aorta inner-diameter and HDL-C are negatively correlated with the number of flow voids, while the content of serum uric acid is positively correlated with the number of flow voids. In the multiple regression analysis,high-density lipoprotein cholesterol(β=9.613,95%CI[0.736-18.490],p=0.034)and the ascending aorta innerdiameter(β=1.019,95%CI[0.312-1.725],p=0.005)were positively correlated with the area of flow voids;high-density lipoprotein cholesterol HDL-C(β=429.295,95%CI[113.290-745.299],p=0.009)is positively correlated with the size of the flow voids;the ascending aorta inner-diameter(β=-128.853,95%CI[-401.635--36.071],p=0.020)is negatively related to the number of flow voids. The differences are all significantly. Conclusion: The increase of high-density lipoprotein cholesterol level will be accompanied by the decrease of choroidal capillary blood flow in hypertensive patients. The increase in the inner diameter of the ascending aorta will be accompanied by a decrease in the blood flow of the choroidal capillaries in hypertensive patients. There is a connection between the choroidal capillary blood flow and the risk factors of hypertension-related cardiovascular diseases.
