Objective: Analyze the stone composition of 299 patients with upper urinary tract stones admitted to the Department of Urology,the First Affiliated Hospital of China Medical University,illustrate the compositional characteristics of upper urinary tract stones in Shenyang and surrounding areas,and observe the effect of the prevention recommendations based on the analysis of the stone composition for different components.Method: Collect patients with upper urinary tract stones who underwent surgical treatment at the First Affiliated Hospital of China Medical University from October 2017 to March 2019.After strict inclusion and exclusion criteria were implemented,299 patients with upper urinary tract stones were collected.Perform data analysis on clinical data and stone composition analysis results.Based on the results,the patients are given corresponding preventive recommendations and the patients are asked to review regularly.The patients were followed up by telephone to ask about the implementation of the recommendations.Those who strictly obeyed the recommendations were in the experimental group,and those who did not strictly obeyed the recommendations after the operation were in the control group.Result: Analysis of stone composition showed that most upper urinary tract stones in Shenyang and surrounding areas were mixed stones,accounting for 71.2% of the total.Calcium oxalate monohydrate(86.3%),calcium oxalate dihydrate(66.6%),and carbonate apatite(51.8%)are the three most common components of upper urinary tract stones in Shenyang and surrounding areas.Analyzing the relationship between each type of stone composition and gender,then we find that carbonated apatite is more common in female kidney stones(p=0.000<0.05),while the difference in the detection rate of other components is not statistically significant.Divide various types of stones into six categories,and divide patients into four groups according to age.The statistical analysis of the six types of stones in each age group showed that the incidence of the six types of stones in each age group was not exactly the same(p=0.009<0.05;p=0.000<0.05;p=0.000<0.05;p=0.000<0.05).The six types of components were compared between age groups.Except for cystine stones(p=0.020<0.05),the detection rates of other types of stones were not statistically different in different age groups(p=0.321>0.05;p=0.676>0.05;p=0.299>0.05;p=0.059>0.05;p=0.583>0.05).There were 213 cases in the experimental group and 86 cases in the control group.The stone recurrence rate in the experimental group(6.57%)was lower than that in the control group(13.95%),which was statistically significant(p=0.040<0.05).The recurrence rate of calcium oxalate stones(p=0.018<0.05)and calcium phosphate stones(p=0.025<0.05)in the experimental group was lower than that in the control group.Conclusion: Calcium oxalate stones and carbonated apatite are the main components of upper urinary tract stones in Shenyang and surrounding areas,and they often appear in the form of mixed stones.Carbonated apatite is more often detected in female patients with upper urinary tract stones.Prevention recommendations based on the results of the analysis of stone components can significantly reduce the recurrence rate of upper urinary tract stones,especially for calcium oxalate stones and calcium phosphate stones,which are worthy of further promotion in clinical applications.
