| Objective : To observe the clinical efficacy of acupotomy combined with acupuncture in the treatment of lower limb muscle spasm and walking ability in the convalescent stage of stroke,so as to provide ideas and reference for clinical treatment of lower limb spasm in the convalescent stage of stroke.Methods : 50 patients with lower limb spasm in convalescent stroke were randomly divided into two groups: Needle knife combined with Acupuncture Group(Experimental Group)and simple acupuncture group(control group),each group had25 cases,acupuncture and moxibustion according to "acupuncture and moxibustion" fifteen planning teaching materials to select points with different techniques(strengthening,weakening),the control group selected points with the test group,the use of techniques flat reinforcing and reducing method,continuous treatment for 4courses,the activities of daily Living(ADL),lower limb Fugl-Meyer Joint Movement score(FMA),spasticity classification(Mas)and gait analysis were recorded and analyzed before and after treatmentResults:1.Activities of daily Living(ADL): After treatment,the activities of daily living(Adl)of both groups were significantly higher than that before treatment(P <0.05),there was a statistical difference between the two groups after treatment(P <0.05),indicating that the acupuncture-knife combined with acupuncture group was superior to the simple acupuncture group.2.Fugl Meyer range of motion(FMA): compared with before treatment,the lower limb range of motion scores of the two groups were improved,the difference was statistically significant(P < 0.05),indicating that the two groups can improve the lower limb range of motion of patients with spasticity,and the difference was statistically significant(P < 0.05)compared with the control group after treatment,indicating that the experimental group is better than the control group in improving the lower limb range of motion of patients with spasticity.3.Spasticity classification: compared with before treatment,the lower limb spasticity classification of the two groups was lower than before treatment,and the difference was statistically significant(P < 0.05),indicating that both groups could relieve the lower limb spasticity of patients,and the difference was statistically significant compared with the control group after treatment(P < 0.05),indicating that the experimental group was better than the control group in relieving muscle spasticity.4.Gait analysis: compared with before treatment,the stride frequency,stride length,stride speed of the two groups were improved,the step deviation was reduced,and the Tinetti score was improved,with statistically significant difference(P < 0.05),indicating that the two groups can improve the gait of patients;(3)compared with the control group after treatment,the difference was statistically significant(P < 0.05),indicating that the experimental group improved the gait of patients better than the control group.5.Clinical efficacy evaluation: the total effective rate of the experimental group was 88%,and that of the control group was 72%.The overall efficacy of the experimental group was better than that of the control group(P < 0.05).Conclusion:acupotomy combined with acupuncture has a better clinical effect in the treatment of lower limb spasticity in the recovery stage of stroke,and is superior to simple acupuncture in the improvement of activities of daily living,motor function,spasticity and gait. |
