| ObjectiveObesity and obesity-related inflammation play an important role in promoting tumorigenesis and development.In recent years,chronic inflammation of breast white adipose tissue(WAT)has caused widespread concern.This study aims to investigate the relationship between breast WAT inflammation with overweight or obesity,menopausal status,tumor characteristics,metabolic syndrome in breast cancer patients,and the correlation between adipocyte diameter and the severity of WAT inflammation and body mass index(BMI).MethodsIn breast adipose tissue,the crown-like structure(CLS-B)formed by macrophages surrounding dying or dead adipocytes is considered to be a marker of breast WAT inflammation.This study selects 136 women with invasive breast cancer diagnosed and treated in Nanjing Drum Tower Hospital from January 2017 to May 2019,with a age distribution of 29-72 years old and a median age of 52 years old.Breast adipose tissues without tumor are collected and formalin fixed and paraffin embedded on the day of surgery,and 8ml fasting blood samples are collected within one week before surgery.Anti-CD68 immunohistochemical staining is performed to identify CLS-B,and the adipocyte size is measured by Hematoxylin and Eosin(H&E)staining.SPSS 24.0 software was used to analyze the data.Results1.The presence of CLS-B is detected in 55(40.4%)patients.The BMI of CLS-B positive patients is higher than that of CLS-B negative patients.The rate of lymph node metastasis is lower in CLS-B positive patients.In the overweight / obese subgroup,CLS-B positive patients are more likely to develop ER-positive breast cancer.2.Compared with normal BMI patients,the CLS-B positive rate of overweight / obese patients is higher.CLS-B density of overweight / obese patients is higher than that of normal BMI patients.CLS-B density is linearly positively correlated with BMI.3.The average adipocyte diameter of CLS-B positive patients is larger than that of CLS-B negative patients.In the normal BMI subgroup,the average adipocyte diameter of CLS-B positive patients is also larger than that of CLS-B negative patients.The average adipocyte diameter is linearly positively correlated with CLS-B density.4.Compared with normal BMI patients,the average adipocyte diameter of overweight /obese patients is larger.The average adipocyte diameter is linearly positively correlated with BMI.5.Postmenopausal women are more likely to suffer from severe WAT inflammation.Subgroup analysis showed that overweight / obese postmenopausal women have the highest CLS-B positive rate.The average adipocyte diameter of overweight / obese postmenopausal women is the largest.6.Compared with CLS-B negative patients,CLS-B positive patients have higher levels of serum triglycerides(TG),total cholesterol(TG),low-density lipoprotein cholesterol(LDL-C),and apolipoprotein B(Apo-B),and higher levels of serum glucose and CRP.In the normal BMI subgroup,CLS-B positive patients have higher levels of serum TC,LDL-C,Apo-B and glucose.ConclusionThe results show that breast WAT inflammation usually occurs in overweight / obese breast cancer patients,and overweight / obese WAT inflammation patients are more likely to develop ER-positive breast cancer.Metabolic syndrome and its components may promote the development of breast cancer through WAT inflammation. |
