Objective The Lorenz scatter plot characteristics of paroxysmal atrial fibrillation(PAF)with different loads were analyzed and summarized.Thus,PAF can be quickly identified and its detection rate can be improved;At the same time,it discusses the characteristics of heart structure function indexes and heart rate variability indexes in the graphic characteristics of each group of Lorenz scatter plots to better guide clinical work.Methods A collection of 95 patients diagnosed as paroxysmal atrial fibrillation in the outpatient and inpatient departments of our hospital from October 2019 to November 2020 as the atrial fibrillation group and 99 healthy controls(excluding arrhythmia by dynamic electrocardiogram).The patients were divided into three groups according to the different amount of load of atrial fibrillation:Group A include 48 cases,0 < load ≤33%;18 cases in group B,33% < load ≤ 66%;Group C include 48 cases,66% < load < 100%.The morphological characteristics of Lorenz scatter plots of paroxysmal atrial fibrillation can be summarized into three types:fan-shaped,umbrella-shaped,and exclamation mark-shaped.The characteristics of three types of Lorenz scatter plots are analyzed.And we compare and summarize the distribution rules of three kinds of scatter graphs in group A,B and C.Cardiac structural and functional indexes(LAD,RAD,LVEDD,LVESD,LVEF,E/A,CO)and heart rate variability(SDNN,HF,LF,LF/HF)were compared between atrial fibrillation groups and control group.P<0.05 was considered statistically significant.Results1.The composition ratio of the three types of graphics features: the fan-shaped accounted for 33.7%,the umbrella-shaped accounted for 25.2%,the exclamation mark-shaped accounted for 41.1%,and the exclamation mark-shaped accounted for the highest proportion.Fan-shaped was most common in group C,exclamation mark-shaped was only found in group A.Umbrella-shaped appeared in both group A and group B,but it was more common in group B.2.The indexes of left atrial diameter,left ventricular end-diastolic dimension and left ventricular end-systolic dimension increased gradually with the increase of the load of each pattern characteristic in the PAF group.The indexes of left ventricular ejection fraction and cardiac output decreased gradually with the increase of the load of each pattern characteristic in the PAF group.The severe abnormality of E/A indexes was most common in the fan-shaped atrial fibrillation group.3.In exclamation mark-shaped atrial fibrillation group,SDNN decreased and LF/HF increased,indicating sympathetic atrial fibrillation.In umbrella-shaped and fan-shaped atrial fibrillation groups,SDNN increased and LF/HF decreased,indicating vagal atrial fibrillation.Conclusions The features of Lorenz scatter plot can reflect the load of paroxysmal atrial fibrillation,the structure and function of the heart,and the type of autonomic nerve.
