Effect Of Modified Yunqi Powder Fumigation Combined With Acupuncture On Oxidative Stress Of Diabetic Peripheral Neuropathy With Deficiency Of Spleen And Kidney And Blood Stasis

Objective To observe the effect of acupuncture combined with fumigation on clinical efficacy,laboratory indexes and serum oxidative stress indexes(MDA,SOD)in patients with diabetic peripheral neuropathy(DPN),So as to explore the clinical mechanism of DPN(Spleen kidney deficiency with blood stasis)by this method.Methods 132 outpatients or inpatients with DPN diagnosed in Yinchuan Hospital of traditional Chinese Medicine Affiliated to Ningxia Medical University from August 2019 to August 2020 were selected and randomly divided into group A,group B,group C and group D,33 cases in each group.After health education,diet and exercise guidance,and basic treatment with western medicine for lowering blood sugar,blood pressure and lipids,group A was given distilled water fumigation,group B was given Yunqi powder fumigation,group C was given acupuncture,and group D was given Yunqi powder fumigation combined with acupuncture.The patients were treated once a day for 2 weeks.Records of the patient’s general condition.Clinical indexes were recorded and serum samples were collected before and after treatment.Motor nerve conduction velocity(MNCV)was measured by electromyography and evoked potentiometer,sensory nerve quantitative detector is used to inspect current sensory thresholds(CPT),serum malondialdehyde(MDA)concentration was determined by ELISA,using colorimetry method(WST-1)to detect serum superoxide dismutase(SOD)activity.Result1.Comparison results of baseline data: there were no significant differences in gender,age,DM course,DPN course,Hb A1 c and BMI among all groups(P<0.05).2.Efficacy evaluation results: after treatment,the total effective rate of the distilled water fumigation group was 38.7%,the total effective rate of the Yunqi powder fumigation group was 78.8%,the total effective rate of the acupuncture group was 79.3%,and the total effective rate of the Yunqi powder fumigation combined with acupuncture group was 90.0%.Compared with distilled water fumigation group,the total effective rate of Yunqi powder fumigation group,acupuncture group and Yunqi powder fumigation combined with acupuncture group were all higher than that of distilled water fumigation group(P<0.01).There was no statistical significance in the total effective rate between the Yunqi powder fumigation group and the acupuncture group(P>0.05).The total effective rate of Yunqi powder fumigation combined with acupuncture group was higher than that of the other three groups(P<0.05).The results indicated that the fumigation of Yunqi powder combined with acupuncture had the best overall curative effect among the treatment groups.3.Score comparison results: Compared with before treatment,TCM syndrome score and TCSS score in 4 groups were significantly decreased after treatment(P<0.01);After treatment,the scores of Yunqi powder fumigation group,acupuncture group and Yunqi powder fumigation combined with acupuncture group compared with distilled water fumigation group were significantly different(P<0.05).After treatment,there were no significant differences in TCM syndrome score and TCSS score between Yunqi powder fumigation group and acupuncture group(P>0.05).After treatment,the decrease degree of scores in Yunqi powder fumigation combined with acupuncture group was higher than that in the other three groups(P<0.05).It indicated that the scores of the 4 groups of patients could be reduced,and the Yunqi powder fumigation combined with acupuncture group was better than the other 3 groups in improving the symptoms of patients.4.Results of MNCV in the peroneal nerve: Compared with before treatment on the same side,nerve conduction velocity in the 4 groups was significantly increased after treatment(P<0.01);After treatment,MNCV in Yunqi powder fumigation group,acupuncture group and Yunqi powder fumigation combined with acupuncture group were significantly higher than that in distilled water fumigation group(P<0.05).There was no significant difference between the Yunqi powder fumigation group and the acupuncture group(P>0.05).After treatment,the fumigation of Yunqi powder combined with acupuncture increased MNCV,and there was statistical significance in comparison with the other three groups(P<0.05).It indicated that MNCV of bilateral common peroneal nerve was increased in the 4groups after treatment,and the Yunqi powder fumigation and acupuncture group had more obvious advantages in improving nerve conduction velocity than the other 3 groups.5.CPT results of superficial deep peroneal nerve: Compared with before treatment on the same side and at the same frequency in this group,the current sensory threshold in 4groups was significantly decreased after treatment(P<0.01);After treatment,CPT decreased significantly in Yunqi powder fumigation group,acupuncture group and Yunqi powder fumigation combined with acupuncture group compared with distilled water fumigation group(P<0.05).After treatment,there was no significant difference in CPT between the Yunqi powder fumigation group and the acupuncture group(P>0.05).After treatment,the CPT of Yunqi powder fumigation combined with acupuncture group decreased significantly compared with the other three groups(P<0.05).The results indicated that the CPT of the superficial deep peroneal nerve decreased at different frequencies on both sides in the 4groups after treatment,and the decrease of the current sensory threshold of the superficial deep peroneal nerve in the Yunqi powder fumigation combined with acupuncture group was better than that in the other 3 groups.6.MDA concentration and SOD activity results: Compared with before treatment,the MDA concentration level of the four groups decreased significantly(P<0.01)and SOD activity increased significantly(P<0.01);After treatment,there were significant differences in MDA concentration and SOD activity between Yunqi powder fumigation group,acupuncture group and Yunqi powder fumigation combined with acupuncture group compared with distilled water fumigation group(P<0.05).After treatment,there was no significant difference in MDA concentration and SOD activity between Yunqi powder fumigation group and acupuncture group(P>0.05).Compared with the other three groups after treatment,the concentration of MDA in serum decreased and SOD activity increased more significantly in Yunqi powder fumigation combined with acupuncture group(P<0.05).The results indicated that the serum MDA concentration decreased and SOD activity increased in the 4 groups after treatment.Compared with the other 3 groups,the Yunqi powder fumigation combined with acupuncture group had more obvious advantages in reducing serum MDA concentration and improving SOD activity.Conclusion Yunqi powder fumigation combined with acupuncture in the treatment of diabetic peripheral neuropathy(deficiency of spleen and kidney with blood stasis).It can significantly improve the clinical symptoms,nerve conduction velocity and current sensing threshold of patients.It can reduce or inhibit the oxidative capacity of the body and improve the antioxidant capacity of the body by reducing the concentration of MDA in the serum and increasing the activity of SOD.And then plays a role in repairing damaged nerves and improving clinical symptoms.
