Objective:To understand the clinical characteristics,drug resistance,and influencing factors of severe cases of H.influenzae pneumonia in hospitalized children,and to provide references for the prevention of children with H.influenzae pneumonia and the rational use of antibiotics.Method:Use retrospective research methods.Taking 1738 children hospitalized with pneumonia from July 31,2016 to January 31,2020 in XX Hospital and meeting the inclusion criteria as the research objects,the gender,age,hospitalization time,hospitalization days,chief complaint,and specialty of the cases were collected Clinical information such as examination,lung imaging,blood gas analysis,diagnosis,culture,and drug sensitivity results are divided into severe and non-critical groups according to the condition of the disease.The database is established by Excel,and the Excel table and SPSS software are used to determine the clinical characteristics of the cases.Analyze the influencing factors of,drug resistance and severe illness,and draw a chart for description,and finally summarize the analysis.Result:1、 The clinical features of Haemophilus influenzae pneumonia in hospitalized children were as follows(1).Age distribution: neonatal group 19 cases,accounting for 1.1%;infant group 822 cases,accounting for 47.3%;infant group 545 cases,accounting for 31.4%;preschool group335 cases,accounting for 19.3%;school age group and above 17 cases,accounting for 1%.(2).Gender ratio: 1086 males,652 females,male: female = 1.67:1.(3).The peak of hi pneumonia infection in hospital appeared in December every year,and it was high in winter and spring.(4).Clinical features: 1182 cases had fever,accounting for 68%;1683 cases had cough,accounting for 96.8%;1137 cases had fever and cough,accounting for 65.4%;711 cases had phlegm sounds,accounting for 40.9%;499 cases had wet rales,accounting for 28.7%;667cases had wheezing,accounting for 99.5% There were 38.4%,6 cases of respiratory sound decreased,accounting for 0.3%,184 cases(10.6%)without pulmonary rales,and the average hospitalization time was 7.37 days.(5).Among the 1738 cases,597 cases(34.3%)were complicated with other pathogens,including 259 cases of respiratory syncytial virus,138 cases of Mycoplasma pneumoniae,38 cases of Moraxella catarrhalis and 37 cases of Staphylococcus aureus,accounting for 43.4%,23.1%,6.3% and 6.2% respectively.2、Drug susceptibility of hospitalized children with Haemophilus influenzae pneumonia:(1)The drug susceptibility test results of the study cases in the past 4 years show that the non-susceptibility rate: compound trimethoprim 80.6%,erythromycin 72.3%,ampicillin67.8%,cefuroxime 33.1%,azithromycin 22.3%,ciproxazole Star 1.5%,with a significant increase,the third-generation cephalosporin is basically sensitive,ampicillin/sulbactam is mostly sensitive.(2)In the past 3 years,the insensitivity rate of ampicillin has shown an upward trend.(3)The sensitivity rate of azithromycin in the infant group is as high as 80.6%,and the sensitivity rate of erythromycin in the infant group and preschool group is the high but only39.3% and 40.6%.3 、 Correlation analysis of severe cases of Haemophilus influenzae pneumonia in hospitalized children:The occurrence of severe cases is positively correlated with co-infection.At the same time,resistance to cefuroxime and ceftriaxone will also increase the risk of severe illness;the high incidence of HI is a high risk factor for severe cases and the proportion of severe illness.Conclusion1.The high incidence of hi pneumonia was less than 3 years old,especially in young infants,which was more common in men;winter and spring were the high incidence seasons of hi pneumonia.2.Penicillin,the first and second generation cephalosporins are not suitable for the first-line use,the third generation cephalosporins should be the first choice,and ampicillin /sulbactam also has a high sensitivity rate.3.The high incidence season of hi is the high risk factor of severe cases and the proportion of severe cases;concurrent infection,cefuroxime resistance and ceftriaxone resistance will also increase the risk of severe cases.
