Objective:This study aims to understand the current status of the fear of cancer recurrence in breast cancer patients,analyze the related factors influencing the fear of cancer recurrence in breast cancer patients,and explore the application effects of psychological intervention methods based on mindfulness decompression therapy in the fear of cancer recurrence in breast cancer patients.Methods:Part Ⅰ: Research on the Fear of Cancer Recurrence in Breast Cancer Patients.This part adopts the convenience sampling method to conduct a questionnaire survey of 270 breast cancer patients who met the inclusion and exclusion criteria in the department of oncology and breast surgery in a tertiary first-class hospital in a certain area of Xinjiang from December2019 to August 2020.Use general information questionnaire,Fear of Cancer Recurrence Inventory(FCRI),Pittsburgh Sleep Quality Index(PSQI),Hospital anxiety and depress scale(HADS)And Simple Coping Style Questionnaire(SCSQ)and other research tools to understand the current situation of cancer recurrence fear in breast cancer patients,and analyze the related factors influencing the fear of cancer recurrence in breast cancer patients.Part Ⅱ: Intervention of Mindfulness Stress Reduction Therapy on the Fear of Cancer Recurrence in Breast Cancer Patients.Seventy breast cancer patients who met the inclusion and exclusion criteria were selected from the patients in the current study as the study subjects.SPSS26.0 software random grouping method was used to divide them into experimental group(n=35)and control group(n=35).The control group received routine care,and the experimental group received online mindfulness-based stress reduction therapy for 8 weeks in addition to the blank group.Before and after intervention,general data questionnaire,fear of cancer recurrence scale and Pittsburgh sleep index scale were used to evaluate,to explore the intervention effect of mindfulness decompressor therapy on fear of cancer recurrence in breast cancer patients.Results:Part Ⅰ: Investigation on the Fear of Cancer Recurrence in Breast Cancer Patients1.The total score of fear of cancer recurrence is(84.40±23.14)points,78.7% of patients have moderate or above-moderate fear of cancer recurrence;the total score of sleep quality is9(6,13)points,and 70.1% of patients have sleep disorders(PSQI> 7 points).2.The results of univariate analysis showed that the family history of cancer and the degree of pain were statistically significant in the fear of cancer recurrence in breast cancer patients(P<0.05).3.Correlation analysis: the fear of cancer recurrence in breast cancer patients is positively correlated with sleep quality(r=0.402,P<0.01),and positively correlated with anxiety and depression(r=0.487;r=0.402,both P<0.01),and The negative coping was positively correlated(r=0.314,P<0.01).Part Ⅱ: The intervention results of mindfulness decompression therapy on the fear of cancer recurrence in breast cancer patients1.Before the intervention,there was no statistically significant difference in the general demographic data of the two groups of patients,the scores and total scores of each dimension of cancer recurrence fear,and the scores and total scores of each dimension of sleep quality(P>0.05).2.After the intervention2.1 Comparison between groups(1)The total scores of fear of cancer recurrence in the control group and the experimental group were(86.17±17.14)points and(72.20±15.47)points respectively,the difference was statistically significant(P<0.05);(2)The total sleep quality scores of the control group and the experimental group were(10.50 ± 5.08)points and(7.33 ± 3.75)points respectively,the difference was statistically significant(P<0.05);2.2 Comparison within the group:(1)The total scores of fear of cancer recurrence before and after intervention in the experimental group were(89.33±9.75)points and(72.20±15.47)points respectively,the difference was statistically significant(P<0.05);the total scores of sleep quality before and after intervention in the experimental group were respectively It is(11.87±4.13)points and(7.33±3.75)points,the difference was statistically significant(P<0.05);(2)The total scores of fear of cancer recurrence before and after intervention in the control group were(86.53 ± 8.38)and(86.17 ± 17.14)points,and the difference was not statistically significant(P> 0.05);the total score of sleep quality before and after intervention in the control group was(11.10±4.32)and(10.50±5.08)points,and the difference was not statistically significant(P> 0.05).Conclusions:1.The level of fear of cancer recurrence in breast cancer patients is generally at a moderate or above level.The fear of cancer recurrence is significantly positively correlated with sleep quality,anxiety and depression,and negative coping.The higher the fear of cancer recurrence,the worse the sleep quality of breast cancer patients,which in turn affects patients’ anxiety and depression,and the more prominent the choice of negative coping methods.2.The results of the study show that the implementation of psychological interventions based on mindfulness decompression therapy can effectively reduce the fear of cancer recurrence in breast cancer patients,and improve the sleep quality of breast cancer patients to a certain extent.The method is simple and easy to learn,and the learning cost is low.It can effectively help breast cancer to self-adjust to patients,adjust psychological discomfort,and alleviate sleep problems.
