| Objective:To explore the relationship between cytokines and survival by observing cytokines levels before or after radiotherapy in patients with esophageal squamous cell carcinoma(ESCC).To find hematological indicators that can simply,accurately and effectively predict the total survival and recurrence of patients with esophageal squamous cell carcinoma after radiotherapy,in order to better evaluate the prognosis of patients with esophageal squamous cell carcinoma after radiotherapy.Methods: A retrospective study was conducted on 70 patients with esophageal squamous cell carcinoma who received radical radiotherapy or radical radiotherapy and chemotherapy in the department of radiotherapy of the fourth Hospital of Hebei Medical University from November 2017 to December 2019.The levels of cytokines before or after radiotherapy were recorded.Including interleukin-1 β(IL-1β),interleukin-2(IL-2),interleukin-4(IL-4),interleukin-5(IL-5),interleukin-6(IL-6),interleukin-8(IL-8),interleukin-10(IL-10),interleukin-12p70(IL-12p70),Interleukin-17(IL-17),tumor necrosis factor-α(TNF-α),interferon-γ(IFN-γ)and interferon-α(IFN-α)are 12 kinds of cytokines.SPSS 22.0 software and Graph Pad Prism 8 were used for data processing.The survival rate,progression-free survival rate and local control rate were calculated by Kaplan-Meier method and univariate prognostic analysis.Cox regression model was used to analyze the multivariate prognosis.P<0.05 showed that the results were statistically significant.Using ROC curve analysis,the predictive value of cytokines levels in patients before or after radiotherapy were more accurately evaluated by calculating,measuring and comparing the area under receiver operating characteristic(AUROC)curves.Results: The follow-up date ended on January 31,2021,1 case lost follow-up,and the follow-up rate was 98.6%.The median follow-up time was19.6 months(0.7-31.8).The OS rates of the whole group at 6 months,12 months and 18 months were 87.1%,72.9% and 62.7% respectively,the PFS rates were 78.8%,55.3% and 37.2% respectively,and the LC rates were81.6%,62.5% and 55.9%,respectively.Univariate analysis showed that gender,tumor length,T stage and TNM stage were the influencing factors of OS(P<0.05),and the mode of chemotherapy was the influencing factor of PFS and LC(P<0.05).The results showed that the levels of cytokines before or after radiotherapy had different effects on OS,PFS and LC(P<0.05).The level of IFN-α before radiotherapy,IL-6 and IL-8 after radiotherapy were the influencing factors of OS.The levels of IL-4 before radiotherapy,IL-2 and IL-12p70 after radiotherapy were the influencing factors of PFS.The level of IL-12p70 after radiotherapy was the influencing factor of LC.Further multivariate analysis showed that the level of IL-8 after radiotherapy was the independent factor of OS,the level of IL-2 after radiotherapy was the independent factor of PFS,and the level of IL-12p70 after radiotherapy was the independent factor of LC.Finally,the ROC curve was used to evaluate the predictive value of many indexes for OS,PFS and LC.The results showed that the level of IL-12p70 after radiotherapy was the best index for predicting OS,PFS and LC(AUROC=0.71,AUROC=0.67,AUROC=0.66).Conclusion:1.In addition to the traditional prognostic factors,peripheral blood interleukin 2(IL-2),interleukin 8(IL-8)and interleukin-12p70(IL-12p70)can predict the survival of patients with esophageal squamous cell carcinoma after radiotherapy or chemoradiotherapy.2.The results of small samples showed that the predictive value of after radiotherapy indexes such as IL-2,IL-8 and IL-12p70 were stronger than those before radiotherapy,and the predictive value of IL-12p70 after radiotherapy was the most great.3.This study was a retrospective analysis,the content of this study has just begun,so the number of cases was also relatively small,the reliability of the conclusion need to be confirmed by multicenter prospective large sample studies in the future. |
