Objective:The aim of this paper was to analyze the clinical effect of keratoplasty in the treatment of infectious keratitis and evaluate the clinical value of timely surgical intervention.Methods:The clinical data of 52 patients(52 eyes)with infectious keratitis who received corneal transplantation and failed in conservative treatment from June 2014 to June 2019 were analyzed retrospectively.The clinical curative effect was evaluated by the cure rate,best corrected visual acuity,transparency of corneal implant and postoperative complications.Results:1.The cure rate was 98.1%.1case(1 eye)was finally enucleated due to postoperative infection.The total enucleation rate was 1.9%.2.42 cases(42 eyes)improved the best corrected visual acuity,accounting for 82.4%.The difference was statistically significant before and after operation(χ~2=20.27,P=0.00).There was no significant difference in the improvement of best corrected visual acuity between deep lamellar keratoplasty group and penetrating keratoplasty group(χ~2=1.09,P=0.30).3.39 cases(39 eyes)were completely transparent,accounting for 76.5%.There was no significant difference between deep lamellar keratoplasty group and penetrating keratoplasty group(χ~2=0.30,P=0.59).4.Postoperative complications:immune rejection:There was significant difference between deep lamellar keratoplasty group and penetrating keratoplasty group(χ~2=4.55,P=0.03);Secondary glaucoma:There was significant difference between deep lamellar keratoplasty group and penetrating keratoplasty group(χ~2=4.05,P=0.04);Postoperative infection:There was no significant difference between deep lamellar keratoplasty group and penetrating keratoplasty group(χ~2=0.002,P=0.97).Conclusion:1.Keratoplasty is an effectively treatment for infectious keratitis.2.The improvement of graft transparency and visual acuity after deep lamellar keratoplasty is similar to that of penetrating keratoplasty,but the incidence of secondary glaucoma and immune rejection is low.3.Corneal transplantation should be performed as early as possible in patients with infective keratitis whose conservative treatment is ineffective to avoid missing the best operative time.
