| Objective: To summarize and analyze the demographic,clinical,laboratory,immunological characteristics and risk factors of lupus nephritis(LN)in hospitalized systemic lupus erythematosus patients in our hospital,to provide basis for clinical diagnosis and disease assessment,in order to guide clinical treatment,and finally improve the survival outcome of pediatric systemic lupus erythematosus patients.Methods: Patients diagnosed with p SLE for the first time in our hospital between January 2014 to December 2020 were evaluated,clinical and pathological data were collected and statistical analysis was performed.Results: 1.A total of 156 p SLE patients were analyzed.The male:female ratio was 1:5.5 and the mean age of diagnosis was 11.17±2.77 years.2.The most frequent initial p SLE symptoms were mucocutaneous(85.26%),haematological(71.15%),renal(59.60%),and the most common clinical manifestation was rash(70.51%).3.Arthritis(40.15%)was the most common initial symptom in female children(P<0.05);Male children had higher rates of proteinuria(75.00%),anemia(70.83%)(P<0.05).4.PSLE in all age groups are more common in 7-12 years old(62.18%),and the least common in 0-6years old(5.77%);At diagnosis,the p SLE patients aged 13-18 years presented with a higher number of fatigue(P<0.05);0-6 years and 13-18 years p SLE patients were more frequently presented with weight loss(P<0.05);0-6years p SLE patients was more frequently presented with neuropsychiatric disorders(P<0.05);13-18 years p SLE patients were less frequently presented with hypertension(P<0.05).5.The most common pathological types of pediatric with LN were class IV(50.79%)and class III(19.05%).6.Multivariate logistic regression analysis showed that hypertension was an independent risk factor for LN.7.The frequently of male children with positive ds-DNA and lymphocytopenia were higher(P<0.05);0-6years p SLE patients most frequently rare with lower C4(P<0.05),with more frequently presented with neuropsychiatric disorders(P<0.05);Children aged 13-18 years(64.00%)showed positive anti-cardiolipin antibody(P<0.05).8.Six cases patients(3.85%)died during the first hospitalization.The main causes of death are infection and multiple organ involvement.Conclusions:1.Our study described the clinical features of p SLE in our hospital and demonstrated significant phenotypic heterogeneity of p SLE.2.The most common clinical manifestation in p SLE at the first diagnosis was rash.3.Class Ⅳ LN was the most common pathological type,hypertension was an independent risk factor for LN.4.Infection and multiple organ involvement during the first hospitalization were the causes of death. |
