Objective: To analyze the risk factors affecting the incidence and severity of retinopathy of prematurity(ROP),and to guide the rational and effective prevention and screening of ROP.Methods: The screening results of retinopathy of prematurity in our hospital between June 1,2018 and May 31,2020 were retrospectively analyzed.Infants were divided into ROP group and non-ROP group.The correlation between risk factors and the incidence and severity of ROP was analyzed,and independent risk factors were determined by multivariate logistic analysis.Receiver operating characteristic curve(ROC)was used to determine the cut-off value,and area under curve(AUC)was used to evaluate the predictive value of risk factors.P<0.05 indicated there was statistic difference.Results: Univariate analysis showed that gestational age,birth weight,lowest level of hemoglobin during hospitalization,duration of oxygen and mechanical ventilation,and small for gestational age were all statistically different between the ROP group and the non-ROP group.Multivariate logistic analysis showed that gestational age and duration of mechanical ventilation were independent risk factors for ROP,and the incidence of ROP was higher with younger gestational age or longer duration of mechanical ventilation.The ROC curve revealed that the cut-off value of gestational age was 30.07 weeks,suggesting that the risk of ROP for premature infants whose gestational age less than 30.07 weeks was increased,and the AUC was 0.715,which had a medium predictive value.The cut-off value of mechanical ventilation duration was 15.5 days.The risk of ROP was increased for preterm infants with mechanical ventilation duration more than 15.5 days,and the AUC was 0.674,which had lower predictive value.Conclusion: Gestational age and duration of mechanical ventilation were independent risk factors for ROP in premature infants.Gestational age has a medium predictive value for the occurrence of ROP.The younger the gestational age,the greater risk of ROP premature infants would face.Reducing numbers of premature infants is the key to prevent ROP.The predictive value of mechanical ventilation time on the occurrence of ROP was low.Premature infants suffering longer duration of mechanical ventilation would be confronted with greater risk of ROP.Those infants whose gestational age were less than 30.07 weeks or those mechanical ventilation duration were longer than 15.5 days should receive more attention during ROP screening.In addition,reducing iatrogenic blood loss,and strictly manage the using of oxygen could decrease the incidence of ROP.
