Objective: To analyse the relationship between the level of serum l ipids and the severity and cognitive impairment of Parkinson’s disease(PD).Methods: PD patients who were hospitalized in the Department of Neurology of Hebei General Hospital from January 2017 to December 2 020 were studied.1.The patients were divided into early stage group(Hoehn-Yahr grade 1-2.5)and middle and late stage group(Hoehn-Yahr grade 3-5).We explored the differences between the two gropes of pat ients in age,gender,age of onset,course of disease,form of onset,tota l cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),very-low density lipoprotein cholesterol(VLDLC),apolipoprotein AI(Apo AI),apolipopr otein B(Aapo B),and lipoprotein a(LPa).The blood lipid indexes with significant differences were selected to analyze the correlation between blood lipid indexes and H-Y grade,gender,age,age of onset,course of disease and form of onset.2.According to the Montreal Cognitive Ass essment scale(Mo CA)and Mini-Mental State Examination(MMSE),pat ients with PD were divided into two groups: PD with cognitive impair ment(PD-CI)and PD with normal cognitive function(PD-NC).According to the Hamilton Anexiety Scale(HAMA)and Hamilton Depression Scale(HAMD)scores,PD patients were divided into groups with anxiety and depression and groups without anxiety and depression.The differences in the above-mentioned indexes were explored between those two groups.Results:1.The average age of patients in early PD group was 65.23±9.447 years old,and that in middle and late PD group was 69.49±8.347 year s old.There was significant difference between the two groups(P<0.05).The age of onset in early PD group was 62.44±9.870 years old,and th at in middle and late PD group was 61.86±12.059 years old.There was no significant difference between the two groups.The course of diseas e in the two groups was 2.00(3.25)years and 5.00(7.00)years respect ively,and the difference was statistically significant(P<0.05).2.There were 77 males and 62 females in the early PD group and48 males and 27 females in the middle and late PD group.There was no significant difference between the two groups(P>0.05).There were93 cases of tremor dominant(TD)and 46 cases of nontremor domina nt(NTD)in the early PD group,40 cases of TD and 35 cases of NTD in the middle and late PD group.There was no significant difference i n the form of onset between the two groups(P>0.05).3.The levels of serum TC in early PD group and middle and late PD group were 4.55±1.023mmol/L and 4.22±0.948mmol/L,respectively,and there was significant difference between the two groups(P<0.05).The serum TG content in the early PD group was 1.11(0.76)mmol/L,and the serum TG content in the middle and late PD group was 0.77(0.47)mmol/L.The difference between the two groups was statistically significant(P<0.05).The levels of serum LDL-C in early PD group and middle and late PD group were 2.88±0.743mmol/L and 2.63±0.613 mmo l/L,respectively,and there was significant difference between the two g roups(P<0.05).The serum VLDL-C levels of the two groups were 0.44±0.310mmol/L and 0.34±0.237mmol/L,respectively,and there was signif icant difference between the two groups(P<0.05).The contents of HDL-C,Apo AI,Apo B and LPa in the two groups were 1.24±0.288mmol/L,1.24±0.336mmol/L,1.32±0.236g/L,1.26±0.292g/L,0.74±0.208g/L,0.68±0.134g/L,204.31(380.53)mg/L and 216.70(287.55)mg/L,respectively.There was no significant difference between the groups(P>0.05).4.The levels of serum TC,TG and LDL-C in patients with PD were correlated with H-Y grade,and the correlation coefficients of Spear man were-0.172,-0.323 and-0.209,respectively.There was no correlation between VLDL-C level and H-Y grade.5.There was no significant difference in blood lipid indexes betwee n PD-CI group and PD-NC group.There was no significant difference in blood lipid indexes between PD with anxiety and depression group and PD without anxiety and depression group.The level of TC was correlated with gender,and the correlation coefficient was 0.272.The level of TG was correlated with age,age and course of disease,and the correlation coef ficients were-0.297,-0.140 and-0.313,respectively.The levels of LDL were correlated with gender,age and course of disease,and the correla tion coefficients were 0.178、-0.160、-0.163,respectively.The level of VLDL-C was not correlated with gender,age,age of onset,course of d isease and form of onset.Conclusions:1.The serum levels of TC,TG and LDL-C in patients with PD were negatively correlated with H-Y grade.The higher the H-Y grade,the lower the above-mentioned indexes.The level of VLDL-C in patients with middle and advanced PD was lower than that in patients with early PD,but there was no clear correlation between VLDL-C and H-Y grade.2.Serum HDL-C,ApoAI,ApoB,LPa levels have no significant correlation with the progression of PD patients.3.The serum TC and LDL-C levels of PD patients may be related to gender.Male patients have lower TC and LDL-C levels than female patients;The older the patient,the older the onset age,and the longer the course of the disease,the lower the TG level;The older the patien t,the longer the course of the disease,the lower the LDL-C level.4.TC,TG,LDL-C,HDL-C,VLDL-C,ApoAI,ApoB,LPa levels have no correlation with PD cognitive dysfunction and mood disorders.The blood lipid level affects the motor symptoms of PD patients more,but has no obvious relationship with the appearance of non-motor symptoms.
