| Objective:To study the correlation between atherogenic index of plasma(AIP)and elderly patients with essential hypertension complicated with coronary heart disease,for the early diagnosis and treatment of elderly patients with essential hypertension complicated with coronary heart disease provide help.Methods:A retrospective study was conducted,A total of 192 elderly patients(age≥65 years old)with essential hypertension who were admitted to the department of cardiology of Hebei People’s Hospital from August 2019 to October 2020 were selected(62 males and 130 females).According to the diagnostic criteria of coronary heart disease,Subjects were divided into hypertension group and hypertension combined with coronary heart disease group.There were 109 cases(56.77%)in the group of hypertension complicated with coronary heart disease,including 37 males and 72 females,with an average age of 71.76±6.09 years.There were 83 cases(43.23%)in the hypertension group,including 25 males and 58 females,with an average age of 73.59±7.11 years.Traditional lipid parameters were detected,and nontraditional lipid comprehensive indexes were calculated.The indexes in the two groups were analyzed and compared.Results:Compared with the essential hypertension group,the level of apolipoprotein A1(apo A1)was lower in the hypertension complicated with coronary heart disease group(P<0.05).In contrast,non-traditional lipid parameters,such as the total cholesterol(TC)/ high density lipoprotein-C(HDL-C)levels in theessential hypertension group were higher(P<0.05).low density lipoprotein-cholesterol(LDL-C)/ HDL-C、apolipoprotein B(apo B)/apo A1、lipoprotein combine index(LCI)、AIP in patients with hypertension complicated with coronary heart disease were higher than those in patients with hypertension alone(P<0.05).The results of Spearman correlation analyses indicated that AIP was positively and significantly correlated with body mass index(BMI)(r=0.215,P=0.003)、 non-high density lipoprotein-cholesterol(Non-HDL-C)(r=0.251,P<0.01)、 atherosclerosis index(AI)(r=0.697,P<0.01).In the univariate logistic regression analysis,traditional lipid parameters such as triglyceride(TG)and apo A1 were associated with the elderly patients with essential hypertension complicated with coronary heart disease(P<0.05).TG was a risk factor for hypertension complicated with coronary heart disease(OR: 1.848,P=0.025),apo A1 was a protective factor for hypertension complicated with coronary heart disease(OR:0.082,P<0.001).In the non-traditional lipid parameters,TC/ HDL-C was negatively correlated with hypertension complicated with coronary heart disease(OR: 0.103,P=0.036).LDL-C/ HDL-C(OR: 1.468,P=0.049)、apo B/apo A1(OR: 6.738,P=0.021)、LCI(OR: 1.034,P=0.016)、AIP(OR: 4.101,P=0.019)were positively correlated with hypertension complicated with coronary heart disease.Further multivariate Logistic regression analysis showed that only AIP was positively correlated with coronary heart disease in elderly patients with essential hypertension(OR: 4.219,P=0.021),which was an independent risk factor for hypertension complicated with coronary heart disease.Conclusion:1.AIP is positively and significantly correlated with BMI、Non-HDL-C and AI.2.AIP was positively correlated with coronary heart disease in elderly patients with essential hypertension. |
