【Objectives】Pulmonary nodule is a common clinical problem in pulmonary diseases.CT-guided percutaneous transthoracic needle biopsy(PTNB)is a minimally invasive technique for lung biopsy.During the operation,it is technically difficult to insert and fix the puncture needle into the lesion stably and accurately,especially for patients with thin chest wall tissue.Therefore,we developed a detachable lung puncture fixation clamp to verify the feasibility of PTNB on a swine lung nodule model.【Methods】(1)Process of building a swine lung nodule model: In general,20 lung nodule models were established in the lungs of living swines using this method.Mixing black ink into ground pork to make black meat paste,then using a syringe to inject the paste into the swine lung through a coaxial puncture needle.Using CT to verify the formation of a solid nodule with a diameter of approximately 1 cm in the swine lung.(2)Applying the detachable lung puncture fixation clamp on the animal experiments: In the beginning,fixing the positioning grid on the surface of the swine’s thoracic body.After determining the puncture point and puncture path by CT,inserting the coaxial puncture needle into the chest wall,and engaging the puncture needle with the detachable lung puncture fixation clamp.Then confirming the spatial relationship and distance between the needle tip and the focus by CT.After the needle reached the surface of the lesion,using a semi-automatic biopsy gun to take the biopsy.During the experiments,recording the adjustment times of the puncture needle into the lung,the average puncture time(the difference between the time of removing the coaxial puncture needle and the scanning time of the positioning image),pneumothorax,pulmonary hemorrhage,CT scanning times,etc.【Results】(1)Swine lung nodule model: Research team successfully modeled 20 swine lung nodules,and CT showed each nodule model with a diameter of approximately1 cm is distinctly separated from the surrounding lung tissue.Our team executed the swain,took out the swine lung,and sent the isolated swine lung nodule model for pathological examination.The examination confirmed that the successfully modeled lung nodule is distinctly separated from the surrounding swine lung tissue.(2)Applying the detachable lung puncture fixation clamp on the animal experiments: In animal experiments,the puncture success rate was 100%,the average puncture time was 13 minutes,the incidence of small pneumothorax was5%,the incidence of pulmonary hemorrhage was 20%,and the average number of CT scans was 6 times.During the experiments,the coaxial puncture needle was fixed in the chest wall stably when it didn’t enter the lung tissue.There was no deviation of the steering shaft when the needle was pushed forward.【Conclusions】41.The swine lung nodules formed by black pork paste can exist stably and distinctly divided from the surrounding lung tissue.2.The clamped type lung puncture fixation device has been tested by animal experiments.As the result of animal experiments,it is safe,feasible and can be used for PTNB. |
