| ObjectiveTo explore the influence of nursing intervention based on Precede-Proceed model on the level of knowledge and practice, exercise endurance, degree of dyspnea and quality of life in elderly COPD patients with pulmonary rehabilitation. It provides new ideas and reference basis for further improving the level of knowledge, belief and practice of pulmonary rehabilitation in elderly COPD patients.MeyhodsA total of 84 elderly patients with COPD who received treatment in the Department of Respiratory Medicine of a Class A hospital in Inner Mongolia Autonomous Region from November 2019 to December 2020 were selected by convenience sampling method, and randomly divided into intervention group and control group, with 42 cases in each group.Patients in both groups received routine nursing care and discharge follow-up.In the intervention group, nursing intervention program based on Precede-Proceed model was carried out at the same time to improve patients’ knowledge and practice level of pulmonary rehabilitation exercise, thus improving patients’ dyspnea degree and exercise endurance, and improving patients’ quality of life. This study was continued to 3 months after discharge to compare the differences in the level of knowledge, belief and practice of pulmonary rehabilitation exercise, degree of dyspnea, exercise endurance and quality of life between the two groups before intervention,at discharge,1 month after discharge and 3 months after discharge.SPSS25.0 software was used for statistical analysis of the experimental data, and P<0.05 was considered to be statistically significant.Results(1)There was no significant difference in general information between the two groups(P>0.05).(2)After the intervention, the scores of knowledge dimension in the intervention group at discharge,1 month and 3 months after discharge were higher than those in the control group(P<0.05);The scores of attitude dimension in the intervention group at discharge,1 month and3 months after discharge were higher than those in the control group(P<0.05);The scores of behavior dimension in the intervention group at discharge,1 month and 3 months after discharge were higher than those in the control group, and the differences were statistically significant(P<0.05).(3)Comparison of dyspnea score between the two groups before and after the intervention: after the intervention, the dyspnea score of the intervention group was significantly lower than that of the control group at 1 month and 3 months after discharge, with significant difference(P<0.05).(4)Comparison of exercise endurance before and after the intervention: after the intervention, the intervention group was higher than the control group in 1 month and 3months after discharge, the differences were statistically significant(P<0.05).(5)Comparison of quality of life scores between the two groups before and after the intervention: after the intervention, except for cough, expectoration and housework, the scores of other items in the intervention group were lower than those in the control group one month after discharge, and the difference was statistically significant(P<0.05);Three months after discharge, the total score and items of cat in the intervention group showed a downward trend, and the scores of chest tightness, asthma, going out, sleep and energy in the intervention group were lower than those in the control group, and the differences were statistically significant(P<0.05).Conclusion(1)Nursing intervention based on Precede-Proceed model can improve the level of knowledge, belief and practice of pulmonary rehabilitation in elderly COPD patients.(2)Nursing intervention based on Precede-Proceed model can improve the degree of dyspnea and exercise endurance of elderly COPD patients,so as to improve the quality of life of patients. |
