Objective:To observe the clinical efficacy,adverse reactions and recurrence rate of the1064 nm PicoWay picosecond laser in the treatment of patients with melasma of different skin types;and provide scientific treatment parameters.Methods: Recruited 20 female patients who were diagnosed with melasma in the Dermatology Clinic of Baotou Central Hospital from January 2019 to May 2020.The screening age was 27-56 years old and the disease course was 1-10 years.According to Fitzpatrick According to the skin type,7 cases(35%)of type Ⅲ skin and 13 cases(65%)of type IV skin.The 20 selected patients were treated with PicoWay picosecond laser once every4 weeks for a total of 3 times.After setting the treatment parameters according to the type Ⅲ and IV skin types,the treatment was performed with Zoom flat hand moments and Resolve lattice hand moments,and each hand moment was treated twice.Follow up before each treatment and 4 weeks and 12 weeks after the third treatment,record all parameters and data,and take facial photographs before each laser treatment and 4 weeks and 12 weeks after the third treatment Keep the files.Two experienced and fixed dermatologists will simultaneously perform the Melasma Area and Severity Index(MASI)scores to obtain the data.If there are objections,the third dermatologist will evaluate and finally reach a consensus.result.The results and conclusions were obtained after statistical analysis of all data using SPSS22.00 statistical system with paired-sample t-test.Results: Before treatment and after 3 laser treatments of 20 female patients,the MASI score before the first treatment was(6.37±2.98),the MASI score before the second treatment was(5.12±2.41),and the MASI score before the second treatment was(5.12±2.41).The MASI score before treatment was(3.96±2.05),and the MASI scores at 4 and 12 weeks after the third treatment were(2.61±1.66)and(2.91±1.43),respectively.The decrease rate of MASI score obtained at 4 weeks after the third treatment was 59%,and the decrease rate of MASI score obtained at 12 weeks after the third treatment was 54.32%;among them,the MASI scores before the first treatment and 4 weeks after the end of the 3 treatments Compared with the scores,the difference was statistically significant(P<0.05);the MASI score at 4 weeks after 3treatments and the MASI score at 12 weeks after 3 treatments were lower than the MASI scores before treatment,and the difference was statistically significant(P <0.05);The MASI score at 12 weeks after 3 times of treatment is also statistically significant compared with the MASI score at 4 weeks after 3 times of treatment(P<0.05);the effective rate of patients at 4 weeks after 3 times of treatment is 65%;The patient’s satisfaction rate for 4 weeks after 3 treatments is 90%.During the course of treatment,all patients only experienced short-term erythema,and no obvious adverse reactions such as edematous erythema,blisters,scabs,pain,post-inflammatory pigmentation,etc.occurred.Conclusion: The 1064 nm PicoWay picosecond laser is safe and effective in the treatment of melasma.The recent recurrence rate is low,the adverse reactions are relatively few,and the time delay is very short.It is worthy of clinical application.
