Clinical Observation Of Yishen Huayu Decoction On Chronic Uric Acid Nephropathy Of Spleen And Kidney Qi Deficiency And Blood Stasis Internal Obstruction

Objective: To observe the clinical effect of Yishen Huayu Decoction on the changes of serum uric acid,serum creatinine,blood urea nitrogen,urinary β2-micro globulin,24 h urine total protein quantity and traditional Chinese medicine syndrome in patients with chronic uric acid nephropathy,so as to provide new ideas and new methods for the treatment of chronic uric acid nephropathy with integrated traditional Chinese and western medicine.Methods: Select a total of 60 patients with chronic uric acid nephropathy who met the inclusion criteria from the Department of Nephrology,Shijiazhuang Hospital of Traditional Chinese Medicine,and divide 30 of them into control group and the rest into treatment group,using the random number table method.Basic treatment was applied in both groups including health education,diet control,drinking more water,alkalizing urine and other treatments.The control group was given febuxostat orally,besides which,the treatment group was given Yishen Huayu Decoction.The course of treatment in both groups lasted 4 weeks.Observe the serum uric acid,serum creatinine,blood urea nitrogen,urinaryβ2 micro globulin,24 h urine protein total quantity,TCM syndrome scores and the changes in clinical disease efficacy of patients before and after treatment,and monitor the safety indexes such as blood routine,liver function and electrocardiogram of patients.The relevant data were statistically analyzed by IBM SPSS 25.0 to evaluate the clinical efficacy and safety of Yishen Huayu Decoction.Results:(1)Comparing the general information(gender,age,course of disease)of the two groups of patients,there was no significant difference(P>0.05),which showed comparability.(2)Comparison of laboratory indicators: Before treatment,there was no significant difference in serum uric acid,serum creatinine,blood urea nitrogen,urinary β2-micro globulin,24 h urine protein total quantity between the two groups with comparability(P>0.05).Compared to that before treatments,the indexes of both groups decreased after treatment.The SUA,SCr,BUN,urinary β2-MG and 24 h UTP in the treatment group significantly decreased after treatments(P<0.01).And the comparison between the groups showed that the laboratory indexes of SUA,SCr,BUN,urinary β2-MG and 24h-UTP in the treatment group decreased significantly than those in the control group,and the difference was statistically significant(P<0.05),the effect of the treatment group is better than that of the control group.(3)Comparison of disease curative effect: The total effective rate of the control group was 80.00%,and the other treatment group was 93.33%.And there are statistical differences between the two groups(P<0.05).Compared to the control group,the treatment group had better efficacy.(4)Comparison of total efficacy and integral of TCM syndrome: The total effective rate of the control group was 86.67%,and that of the treatment group was 93.33%.And there are statistical differences between the two groups(P<0.05).The curative effect of the treatment group was better than that of the other group.The results show that the total scores of TCM syndromes in the two groups were lower than that before treatment,and the difference was statistically significant(P<0.05).Compared with the total scores of TCM syndromes in the two groups,the average score of the control group after treatment was(10.50±3.94).The average score after treatment in the group was(6.97±4.28),the difference between the two groups was statistically significant(P<0.01),and the treatment group was better than the control group.After treatment,the individual symptom scores of the two groups were significantly lower than that before treatment(P<0.01).The score of waist and knee pain,poor appetite,frequent urination at night and joint stinging in the treatment group decreased significantly than that of the other group(P<0.01).There were no statistically significant differences in the individual syndrome scores of fatigue,feel dizzy,tinnitus,numbness of the limbs,and skin nail errors(P>0.05).(5)Safety analysis: There were no adverse reactions occurred in the two groups before and after treatments,and no abnormalities were found in blood routine,liver function and ECG results,indicating that both treatment methods have good safety.Conclusion: Yishen Huayu Recipe is effective in treating chronic uric acid nephropathy with deficiency of spleen and kidney qi and blood stasis,which can reduce SCr,BUN,urinary β2-MG and 24 h UTP indicators.Effectively improve the clinical symptoms of patients without obvious adverse reactions,combined with western medicine treatment has the advantage of improving the efficacy,and is safe and reliable,can be extended to clinical treatment.
