BackgroundThe situation of prevention and control of chronic diseases in China is becoming increasingly serious.Chronic diseases have become the main threat to the health of residents and seriously affected the quality of life of patients.Due to long-term and high burden of chronic diseases,patients are prone to become anxiety.The relatives will also face great pressure during the treatment and rehabilitation of patients.When the quality of life of relatives decreases,it will not only affect their own quality of life,but also affect the quality of care of patients,leading to the decline of patients’ quality of life,forming a vicious circle.However,previous studies mostly focused on patients with chronic diseases.The influence of relatives’ anxiety on the quality of life of patients with chronic diseases was not discussed.Therefore,it is of great significance to pay attention to the impact of anxiety on the quality of life of patients with chronic diseases and their relatives.ObjectivesTo understand the status of anxiety and quality of life of patients with chronic disease and their relatives,and explore the relationship between anxiety and quality of life.The actor-partner interdependence model was used to analyze the impact of anxiety on the quality of life of patients with chronic diseases and the primary caregivers.Providing a basis for formulating measures to improve the quality of life of patients with chronic diseases and their relatives.MethodsThe method of multi-stage sampling was adopted to randomly select 2 towns from 28 towns in Dongguan,and 6 and 10 villages from the selected towns were selected respectively in proportion to the population size.A total of 1775 patients with chronic diseases and their relatives were selected as objects of the research.The trained investigators who adopted the way of household survey conducted face to face inquiry survey to the residents.A self-designed questionnaire was used to collect information on age,gender,educational level,marital status,employment status and chronic disease of the objects.Using the SF-12v2 scale and self-rating anxiety scale to evaluate the quality of life and anxiety of the patients and their relatives respectively.Pearson correlation analysis was used to explore the relevance between anxiety and quality of life in patients with chronic diseases and their relatives.A total of 270 patient-primary caregiver dyads were selected.The Amos24.0 was used to build the actor-partner interdependence model which was used to analyze the impact of anxiety of patients with chronic disease and their primary caregivers on quality of life.Results1.The anxiety rate of surveyed patients with chronic disease was 2.2 %,the anxiety scores of the patients(31.52±6.71)were higher than that of their relatives(27.72±4.16),the difference was statistically significant(P < 0.05).2.The quality of life of the patients with chronic disease(46.36±6.84)was lower than that of their relatives(50.96±5.11),and the difference was statistically significant(P < 0.05).3.The quality of life of patients with chronic disease was influenced by gender,educational level,marital status,employment status,smoking status,the number of physical exercises per week,the number of chronic disease,anxiety of patients(P <0.05);The quality of life of relatives was influenced by age,anxiety of relatives(P <0.05).4.The anxiety of patients with chronic disease was negatively correlated with their own quality of life(r =-0.610,P < 0.001);The anxiety of relatives was also negatively correlated with their own quality of life(r =-0.486,P < 0.001).5.The results of APIM analysis show that,in aspect of actor effect,the anxiety of patients can negatively predict their own total score of quality of life,physical component summary(PCS),general health(GH),physical function(PF),role physical(RP),role emotional(RE),bodily pain(BP),vitality(VT),social function(SF)(P < 0.05);The anxiety of the primary caregivers can negatively predict their own total score of quality of life,physical component summary(PCS),mental component summary(MCS),general health(GH),physical function(PF),role physical(RP),role emotional(RE),bodily pain(BP),mental health(MH),vitality(VT)and social function(SF)(P < 0.05);In the aspect of partner effect,the anxiety score of the primary caregivers can negatively predict mental component summary(MCS),mental health(MH),vitality(VT)of patients(P < 0.05).Conclusions1.The anxiety rate of patients with chronic diseases and their relatives under investigation is relatively low.There are significant differences in the quality of life of patients with chronic diseases and relatives.The influencing factors of the quality of life of patients include gender,education level,marital status,employment status,smoking status,the number of physical exercises per week,the number of chronic diseases and patient’ anxiety;the influencing factors of the quality of life of relatives include age and relatives’ anxiety.2.The anxiety of patients with chronic disease was negatively correlated with their own quality of life,and the anxiety of relatives was negatively correlated with their own quality of life.3.The anxiety of patients with chronic disease and the primary caregivers can negatively predict their own quality of life;The anxiety of the primary caregivers can negatively predict mental component summary,mental health and vitality of patients with chronic disease. |
