Objective:To explore the clinical features and prognostic factors of AFP negative hepatocellular carcinomaMethods:The clinical data of patients with hepatocellular carcinoma who underwent radical hepatectomy in hepatobiliary surgery of Affiliated Hospital of Youjiang Medical College for Nationalities from July 2013 to December 2016 were collected,and the postoperative pathological diagnosis of hepatocellular carcinoma(HCC)was made,The patients were divided into AFP negative group(n = 27)and AFP positive group(n = 104).The gender,age and body mass index(BMI)of the two groups were compared,Objective to analyze the clinical characteristics of AFP negative hepatocellular carcinoma(HCC)patients,and analyze the related factors affecting the survival rate.Results:There was no significant difference in gender(P > 0.050)and age(P = 0.087).The proportion of overweight in AFP negative group was higher than that in AFP positive group(44.5% vs 19.2%,P = 0.016),and the proportion of HBV infection in AFP negative group was lower than that in AFP positive group(40.7% vs 66.3%,P = 0.012);the proportion of small HCC in negative group was higher than that in positive group(22.2% vs5.8%,P = 0.008);AFP negative had fewer satellite nodules and portal vein tumor thrombi than AFP positive(11.11% vs 29.8%,7.4% vs 27.9%,all P < 0.05).The rate of child a in AFP negative group was higher than that in AFP positive group(96.3% vs 79.8%,P = 0.041).The1-year and 3-year survival rates in AFP negative group were higher than those in AFP positive group(96.3% vs 75.0%,59.3% vs 34.6%,P < 0.05).There was no significant difference in the number of tumors and the presence or absence of capsule between the two groups(P =0.097,P = 0.240).Univariate analysis showed that the presence or absence of satellite nodules,liver cirrhosis and degree of differentiation were the three major factors influencing the postoperative survival rate of AFP negative HCC patients(all P < 0.05).Multivariate analysis showed that the degree of differentiation and satellite nodules were independent factors affecting the survival rate of AFP negative HCC patients(P < 0.05).Conclusion:1.Compared with AFP positive HCC patients,AFP negative HCC patients have the characteristics of low HBV infection rate,high proportion of small hepatocellular carcinoma,low child Pugh score,low proportion of liver cirrhosis,few portal vein tumor thrombus and satellite nodules.2.The prognosis of AFP negative HCC is better than that of AFP positive HCC.3.The degree of differentiation and satellite nodules were independent factors affecting the survival rate of AFP negative HCC patients.
