Objective : To explore the prognostic factors of childhood hepatoblastoma(HB)in comprehensive treatment.Methods:The clinical data of 91 children with HB who underwent radical resection in our hospital from June 2011 to October 2019 were retrospectively analyzed.Results:The 5-year tumor-free survival rate(DFS)was 86.7±3.9%;the 5-year overall survival rate(OS)was 92.9±2.8%.AFP of the highest group before and 4 weeks after operation,AFP≥5.48 ng/m L at the end of treatment,and distant metastasis at diagnosis were significantly related to treatment failure.AFP return to normal within 3 cycles of postoperative chemotherapy,thrombocytosis at diagnosis,and early PRETEXT(Ⅰ and Ⅱ)were significantly related to better survival results,and whether AFP dropped 10 times after the first cycle of neoadjuvant chemotherapy,first diagnosed AFP,and age And sex has nothing to do with prognosis.AFP at the end of treatment is used as an independent predictor of prognostic outcome,54.75 ng/m L can be used as a cut-off value for prognostic evaluation.AFP return to normal within 3 cycles of postoperative chemotherapy is more predictive of long-term survival than a 10-fold decrease in AFP after the first cycle of neoadjuvant chemotherapy.Conclusion:Continuous monitoring of the changes in AFP during the perioperative period during diagnosis and treatment can evaluate the chemotherapy sensitivity and long-term prognosis of cancer patients.
