Research On Xiaojianzhong Decoction For Improving Digestive System Malignant Tumor Patients With Loss Of Appetite Symptoms

Objective:In this study,150 patients who are digestive system malignant tumor patients with loss of appetite were studied by the treatment of Xiaojianzhong Decoctionon,and the TCM syndromes,weight,KPS score,FAACT-A/CS,EORCT QLQ-C30,NRS2002 and laboratory indicators were observed before and after treatment.Then,Data mining was conducted on the prescriptions of Chinese medicines of 150 patients with 150 visits,and through the use of medication frequency,association rules,and cluster analysis to explore the rule of medication,about the professor Jia Yantao used Xiaojianzhong Decoction in the clinic to treat patients with loss of appetite of digestive system malignant tumor.Methods:Through inquiries on the computerized medical record system,patients who are digestive system malignant tumor patients with loss of appetite in the outpatient of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from January 2019 to December 2020 were collected accordancing the inclusion and exclusion criteria.Screen the patients treated with Xiaojianzhong Decoction,and through telephone or outpatient obtained the patient’s related symptoms,conducted scale evaluation,collected laboratory related data,compared the differences before and after treatment for evaluating the Xiaojianzhong Decoction’ curative effect;then using SPSS 21.0 and SPSS Modeler 18.0 software conducted data mining on the addition and subtraction drugs except the drug composition of Xiaojianzhong Decoction,and explore the prescription and rules.Results:(1)Xiaojianzhong Decoction can significantly improve thedigestive system malignant tumor with loss of appetite patients’ TCM syndromes scores,KPS scores,weight,FAACT-A/CS scores,NRS2002,there is difference in statistical significance(P<0.05).(2)There were statistically significant differences in the body function,nausea and vomiting,pain,shortness of breath,diarrhea,total health status,fatigue and appetite in the EORTC QLQ-C30 scale,there is difference in statistical significance(P<0.05);there was no significant difference in the part of role function,emotional function,cognitive function,social function,insomnia and economic difficulty,there is no difference in statistical significance(P>0.05).(3)Xiaojianzhong Decoction can improve thedigestive system malignant tumor with loss of appetite patients’ serum albumin,there is difference in statistical significance(P<0.05);but can’t improve serum cholinesterase,hemoglobin in,there is no difference in statistical significance(P>0.05).(4)There was no obvious abnormality in the safety indexes before and after treatment,and no obvious adverse reactions during treatment.(5)In addition to the drugs except the composition of Xiaojianzhong Decoction,the top ten high-frequency drugs are Astragalus,Stir-fried Malt,Radix Ginseng,Oldenlandia diffusa,Poria,Lobelia,Scutellaria barbata,Atractylodes macrocephala,Turmeric,Angelica;divided into Qi-tonifying drugs,spleen-strengthening drugs,heat-clearing and detoxifying drugs.(6)In addition to the drugs except the composition of Xiaojianzhong Decoction,commonly couplet medicines are Astragalus,Atractylodes and Astragalus,Stir-fried Malt or(and)Poria and Astragalus,Lobelia or(and)Scutellaria barbata or(and)Oldenlandia diffusa with Astragalus,Lobelia Or(and)Scutellaria barbata and Hedyotis diffusa.(7)The top 20 traditional Chinese medicines(except the composition of Xiaojianzhong Decoction)can be roughly divided into tonic drugs,digestant drugs,qi-regulating drugs,damp-clearing drugs,drugs for removing blood stasis,and heat-clearing drugs after factor analysis combined with the cluster analysis of the system;F1 consists of Eclipta prostrata,Ligustrum lucidum,Cornus officinalis,Prince Edward Ginseng four-flavored tonic drugs and three heat-clearing drugs of Lobelia,Scutellaria barbata and Hedyotis diffusa,inclouding Turmeric for promoting blood circulation and removing blood stasis.F2 consists of Lobelia and Scutellaria barbata two-flavored heat-clearing medicine and Sargentodoxa cuneata,Patrinia and Turmeric three-flavor medicine for promoting blood circulation and removing blood stasis and stir-fried malt for digestion and spleen-invigorating medicine.F3 is composed of three flavors of Gallus gallus domesticus,Jiao Hawthorn and stir-fried malt for digestion and spleen-invigorating medicine and Radix pseudostellariae for invigorating qi.(8)The four natures of drugs in the 20 medicines are sorted by frequency as flat,warm,cold,lukewarm,slightly cold,and cool;The five flavors of drugs are in the order of sweet,bitter,pungent,slightly bitter,light,sour,and astringent;the meridians are the spleen meridian,Lung meridian,liver meridian,kidney meridian,stomach meridian,heart meridian,small intestine meridian,large intestine meridian,bladder meridian.Conclusion:(1)Xiaojianzhong Decoction can significantly improve the patients with anorexia symptoms of digestive system tumors,The TCM syndrome score,weight,KPS score,FAACT-A/CS score,NRS2002,serum albumin,EORTC QLQ-C30 of body function,nausea,vomiting,pain,shortness of breath,diarrhea,general health,fatigue and appetite can be improve;but serum cholinesterase,hemoglobin,EORTC QLQ-C30 of role functions,emotional functions,cognitive functions,social functions,insomnia,those are no obvious improvement.(2)The top ten high-frequency drugs used in the modified Xiaojianzhong Decoction are Astragalus,Stir-fried Malt,Taizi,Oldenlandia diffusa,Poria,Lobelia,Scutellaria barbata,Atractylodes,Turmeric,Angelica;commonly couplet medicines are Astragalus,Atractylodes and Astragalus,Stir-fried Malt or(and)Poria and Astragalus,Lobelia or(and)Scutellaria barbata or(and)Oldenlandia diffusa with Astragalus,Lobelia or(and)Scutellaria and Scutellaria Tongue grass.The commonly used drugs in compatible drugs are tonic drugs,digestant drugs,qi-regulating drugs,damp-clearing drugs,drugs for removing blood stasis,and heat-clearing drugs.The principle of selection is to use neutral in nature drugs,supplemented by warm drugs to tonify yang and cold drugs to clearing heat.Also regulating the spleen and stomach,Lung,Kidney Channel.
