| Objectives:1.To know the impact of different types of childhood maltreatment on adolescents with depression.2.To predict whether different types of childhood maltreatment is a risk factor for suicidal behavior in adolescents with depression.3.To provide prevention and intervention measures for suicidal behavior of depressed adolescents.Methods:1.105 adolescents admitted to the University-Town Hospital of Chongqing Medical University,those who met the criteria for inclusion in adolescent depression were selected as study subjects.According to the childhood abuse,the no childhood abuse group(52 cases)and the childhood abuse group(53 cases).2.Basic information on adolescent patients with depression who met the inclusion criteria was collected.Hamilton depression scale 17(HAMD-17)and Hamilton anxiety scale(HAMA)were used to assess anxiety and depression.The severity of illness was evaluated by clinical global impression-severity of illness(CGI-SI).Childhood trauma questionnaire-28 item short form(CTQ-SF)was used for assessing childhood trauma.The Beck Scale for Suicide Ideation(BSI)was used to look at suicide.3.T test is used for measurement data meeting the normal distribution,and non-parametric test is used for measurement data not meeting the normal distribution.Enumeration data were intergroup by X~2test.In addition,Pearson correlation analysis was carried out for the scores of HAMD-17,HAMA,CGI-SI,BSI,CTQ-SF scales of adolescent depression patients with childhood maltreatment.Logistic regression analysis was conducted with childhood trauma as the independent variable in the suicide attempt group and the non-suicide attempt group as the dependent variable.Results:1.Our findings were found,there was no significant difference in gender,age,years of education,smoking,one type of childhood abuse,the number of suicidal ideation and suicide attempt between the two groups(P<0.05).While in course of disease,body mass index,alcohol consumption,the number of factors in CTQ-SF,two or more types of childhood abuse,the number of no suicidal ideation and attempted suicide were significant differences between the two groups(P<0.05).2.The HAMD-17,HAMA,BSI score,suicidal ideation and suicide risk scores were compared between the two groups,and the differences were statistically significant(P<0.01).But the CGI-SI score was no statistical significance(P>0.05).3.After the Pearson correlation analysis,it was found that in the childhood maltreatment group,the relationship between them was found,emotional abuse was positively correlated with HAMD-17,HAMA,CGI-SI,BSI total score,suicidal ideation and suicide risk(P<0.01).There was a significant positive correlation between physical abuse and HAMD-17,HAMA and CGI-SI(P<0.01),but no correlation with BSI total score,suicidal ideation and suicide risk(P>0.05).Sexual abuse was a prominent positive correlation with HAMD-17,HAMA,CGI-SI,BSI total score,suicidal ideation and suicide risk(P<0.01).And there was positively correlation between emotional neglect and with BSI total score,suicidal ideation and suicide risk(P<0.01),but no correlation with HAMD,HAMA and CGI-SI(P>0.05).Meanwhile,physical neglect was observably positively related to HAMD-17,HAMA,CGI-SI,BSI total score,suicidal ideation and suicide risk(P<0.01).4.After Logistic regression analysis of childhood maltreatment and suicide attempt in adolescent patients with depression.It was found that the five factors in the attempted suicide section were all included in the Logistic regression equation.Emotional abuse,emotional and physical neglect(P<0.01),physical and sexual abuse(P<0.05),the regression coefficients>0,odds ratio(OR)=Exp(B),>1.That is all the five factors are the risk factors for suicidal attempt in adolescents with depression.Conclusions:1.Adolescents with depression who were abused in childhood had a more severe illness,a longer course of illness,and were more likely to be overweight,drink alcohol,or even obese,excessive alcohol consumption.Most of them probably experienced more than one traumatic event during their childhood,which deserves our attention.2.Adolescents with depression who were abused in childhood were associated with more anxiety and a higher risk of suicide.3.The 5 factors of childhood maltreatment emotional,physical and sexual abuse,emotional and physical neglect in adolescents with depression,especially emotional abuse,sexual abuse and physical neglect,are positively correlated with anxiety and depression,suicidal ideation and suicide risk.4.The 5 factors of childhood maltreatment,emotional,physical and sexual abuse,emotional and physical neglect are all risk factors for suicidal attempted behavior in adolescents with depression.Therefore,the results of this study suggest that the five factors of childhood maltreatment are all risk factors for suicidal behavior in adolescents with depression,which should be paid more attention. |
