Objective: In order to evaluate whether there are differences in microbiome diversity of facial skin between patients with rosacea and healthy controls;and assess the efficacy and safety of minocycline topical preparation in the treatment of the patient with moderate-to-severe papulopustular rosacea.Methods: A case-control study was carried out to contrast the difference of diversity and abundance in facial microorganisms between patients with rosacea and healthy conctrols.Randomized controlled trials of minocycline topical preparation in the treatment of patients with rosacea were collected from the databases of Pub Med,Web of science,Embase and CNKI.Revman5.3 software was used for Meta-analysis to evaluate the efficacy and safety of minocycline topical preparation in the treatment of moderate-to-severe papulopustular rosacea.Results: A total of 22 samples were qualified(including 14 rosacea patients and 8 healthy controls).A total of 493 OUTs,24 phyla were clustered.Compared with the control group,the α diversity in patients with rosacea is higher.The relative abundance decreased including Propionibacterium acnes,Pseudomonas and Streptococcus thermophilus.And the relative abundance increased including Staphylococcus epidermidis,Corynebacterium tuberculostearicum and Neisseriaceae unclassified,but there was no statistically difference in the above microorganisms between the two groups(P>0.05).A total of 3randomized control trials show that minocycline topical preparation could significantly reduce the number of facial inflammatory lesions[MD=4.78,95%CI(1.44,8.13),P=0.005].And more patients can reach IGA 0/1 [RR=1.29,95%CI(1.15 1.45),P<0.0001].In terms of safety,there was no significant difference in the incidence of adverse reactions between minocycline topical preparations and vehicles(P>0.05).Conclusion: Changes of the facial microbial diversity and relative abundance is involved in the pathogenic process of rosacea,but it is not the main cause of rosacea.Minocycline topical preparation is safe and effective in the treatment of moderate-to-severe papulopustular rosacea.
