| Objective This study aimed to explore the clinical characteristics and risk factors of patients with Graves’ disease complicated with pulmonary arterial hypertension(PAH).Methods The clinical data of 138 patients diagnosed with Graves disease in the Endocrinology Department and Nuclear Medicine Department of the First Affiliated Hospital of Chongqing Medical University from January 2013 to July 2019 were collected and retrospectively analyzed.Patients were divided into PAH-free group and PAH group based on echocardiographic results.Application of SPSS 22.0software for statistical analysis,comparing differences in the two groups,analyzing their clinical characteristics and exploring the risk factors of patients with Graves’ disease complicated with pulmonary arterial hypertension(PAH).Results The prevalence of Graves disease combined with PAH was39.9%,with mild to moderate PAH being the main ones.There were no statistically significant differences in gender ratio,age,BMI,hyperthyroidism course,heart rate,and blood pressure between the two groups.The levels of free triiodothyronine(FT3),thyrotropin receptor antibody(TRAb),and triglyceride(TG)in the PAH group were higher than the PAH-free group(P<0.05),and the high-density lipoprotein cholesterol(HDL-C)and total cholesterol(TC)levels were lower than the PAH-free group(P<0.05).FT3,TG are risk factors for PAH in patients with Graves disease,and after establishing 4 models to gradually correct the confounding factors,it is found that TG is the independent risk factor of them(P<0.05).Conclusion The incidence of Graves disease combined with PAH is high,mainly mild to moderate PAH.The differences in FT3,TRAb,TG,HDL-C,and TC levels between the Graves disease combined PAH group and the PAH-free group were statistically significant,but there was no statistical difference in the gender ratio,age,BMI,hyperthyroidism course,heart rate,and blood pressure.The the left atrium,right atrium and right ventricle size of the PAH group were larger than those in the PAH-free group.FT3,TGare risk factors for Graves disease complicated by PAH,and TG is an independent risk factor for Graves disease complicated by PAH.Therefore,for patients with Graves disease complicated with lipid metabolism disorders,especially triglyceride metabolism disorders,it is necessary to closely follow up cardiac color Doppler ultrasound to be alert to the occurrence of PAH. |
