| PurposeThis study collected the clinical data of patients diagnosed with chronic superficial gastritis,then explored the composition and festure of TCM syndromes elements.The association with the symptoms elements and HP infection in patients with this disease was analyzed.The study aimed to provide theory foundation for standardized diagnosis and treatment of chronic superficial gastritis.It was expected that TCM syndrome differentiation could play a certain role in the research of etiology and pathogenesis,treatment and prevention in chronic superficial gastritis.At the same time,this study analyzed and summarized the clinical experience of Chief Physician Song Ping in treating this disease.The experience could guide to TCM cure.MethodThis paper consists of two parts,the first part: according to the inclusion and exclusion criteria,general conditions,living habits,eating habits,symptoms,signs,tongue and pulse conditions and other data of 229 eligible patients with chronic superficial gastritis were surveyed and collected.The TCM Syndrome Differentiation Scale based on the data was completed.The data was sorted and entered using EXCEL 2010,and processed by SPSS21.0to explore the distribution of TCM syndrome elements and the association with the symptoms elements and HP infection in patients with this disease.The second part: this paper summarizes Chief Physician Song Ping’s understanding of chronic superficial gastritis,principles and methods of treatment,empirical prescriptions,etc,and then the diagnosis and treatment experience of Chief Physician Song Ping was summarized based on the case and the experience of daily study with my tutor.Results1.General information:Among the 229 study subjects,the number of male patients was 96(41.9%),the number of female patients was 133(58.1%),and the ratio of male patients to female patients was1:1.39,with more females than males;107(46.7%)young people and 69 middle-aged people.(30.1%),46 cases of young and old people(20.1%),7 cases of old people(3.1%),most of them were young people;in the occupational distribution,75 cases(32.8%)of employees were found to be the most common,followed by retirement(58 retired persons(25.3%);in the distribution of lifestyle habits,30 cases of smoking(13.1%),28 cases of drinking(12.2%),50 cases(21.8%)of patients with irregular diet,88 cases of greasy diet(38.4%);129 patients(56.3%)with normal BMI,57 patients(24.9%)with overweight,24 patients(10.5%)with underweight,19 patients(8.3%)with obesity;distribution of eating habits Among them,the patients who love to eat hot are the most(72 cases,31.4%),and 26.6% of the patients have no diet preference;in the distribution of taste preference,the patients who love sweetness are the most(90 cases,39.3%),followed by spicy food of the patients(78 cases,34.1%),24.5% of the patients had no taste preference;the distribution of sleep status was investigated,and it was found that easy to wake up and sleeplessness were the most(98 cases,42.8%),56 cases of adequate sleep;229 cases of chronic superficiality Among the patients with gastritis,the most often angry patients(86 cases,37.6%),followed by the overthinking patients(83 cases,36.2%);65 patients(28.4%)with anxiety state,164 patients(71.6%)without anxiety state;44of the 229 patients had drug-related disease(19.2%).Among the 229 patients,about half of the patients have no seasonal pattern of onset,and autumn and winter are the main ones among the patients with seasonal pattern.2.Disease location syndrome element distribution:(1)Stomach was the most common disease location(210 cases,91.7%),followed by spleen(57 cases,24.9%),liver(40 cases,17.5%);(2)The distribution of disease locations is mostly single disease patients(144 cases,62.9%),and the combined distribution of disease locations is dominated by double disease patients(64 cases,28.0%),mainly with the same disease of the stomach and spleen,and the same disease of the stomach and liver.3.Distribution of disease-related syndromes:(1)Among the deficiency syndrome elements,Yang deficiency is the most(81 cases,35.4%),followed by Qi deficiency(76 cases,33.2%);among the solid syndrome elements,Qi stagnation is the most(199 cases,86.9%),followed by Wet(83 cases,36.2%);(2)In the distribution of disease combination,the patients with dual disease were the most(72 cases,31.4%),followed by three disease(58 cases,25.3%),and single disease(57cases,24.9%);(3)In the distribution of disease-related deficiency and excess,patients with combined deficiency and excess are the most common,followed by simple empirical evidence,and the least with simple deficiency syndrome.Among them,stagnation of Qi and Yang deficiency is more common in patients with combined deficiency and excess.4.Systematic clustering of the 7 syndrome elements with a frequency of more than 15%.According to the results of the cluster analysis,contacted with the basic theories of TCM and the law of the onset of chronic superficial gastritis,the syndrome elements are grouped into two categories,namely "Spleen Yang Qi deficiency with dampness accumulation" "Stagnation of Liver and Stomach Qi".5.Research on the correlation between various TCM syndromes and HP infection:(1)There is no statistically significant difference between the syndrome factors of each disease location and whether Helicobacter pylori is infected(P>0.05),and there is a statistically significant difference between the "phlegm,dampness,and blood deficiency" of disease syndrome and whether HP is infected(P<0.05),there is no significant difference in whether HP is infected and other syndromes(P>0.05);(2)To explore the correlation between the disease-type syndromes of element dampness,sputum,blood deficiency and HP infection,the results show that the syndrome element dampness is closely related to the positive HP infection.The probability of HP infection positive in patients with syndrome element dampness may be that there is no syndrome element dampness.The number of patients was 4.316 times.6.Summary of diagnosis and treatment experience of Chief Physician Song Ping: the basic pathogenesis of chronic superficial gastritis is summarized as stagnation,dampness and deficiency,with weakness of spleen and stomach as the foundation,stagnation of Qi and internal obstruction of dampness and turbidity as the standard.The treatment focuses on invigorating spleen and regulating Qi,with clearing damp and stomach as the basic prescription,and adding or subtracting the empirical formula according to the syndrome differentiation.Conclusion1.Chronic superficial gastritis is located in the stomach and is nearly connected with the spleen and liver.The research results suggest that the clinical treatment mainly focuses on the stomach,spleen and liver.2.The deficiency syndromes of chronic superficial gastritis are mainly Yang deficiency and Qi deficiency.The excess syndromes of the disease are mainly stagnation of Qi and dampness.3.Chronic superficial gastritis is mostly a mixture of deficiency and excess,among which Qi stagnation and Yang deficiency is the most common.It is suggested that the symptoms and root causes should be treated at the same time during clinical treatment,and the spleen and stomach should be protected.4.The common TCM syndromes of chronic superficial gastritis are: "Spleen Yang Qi deficiency with dampness accumulation" "Stagnation of Liver and Stomach Qi".5.The syndrome elements of the disease site in patients with chronic superficial gastritis have nothing to do with HP infection.The dampness,sputum,and blood deficiency in the disease syndrome elements are related to HP infection;syndrome element dampness is closely related to HP infection positive,and there is evidence Patients with dampness are more likely to be infected with HP.6.Chief Physician Song Ping believes that the key to the cause of chronic superficial gastritis is the deficiency of Yang Qi in the spleen and stomach,stagnation of Qi and internal obstruction of dampness and turbidity.Verification is a dynamic process.The clinic work insists on the treatment of both symptoms and root causes,with the treatment of strengthening the spleen,regulating qi and removing dampness,harmonizing the stomach.Using its empirical formula to add or subtract medication based on syndrome differentiation,it is known that it often changes,and the curative effect is very good.Its experience is worthy of reference and promotion. |
