A Short-term Retrospective Study On The Efficacy Of External Treatment With Ye’s Tuina Of Traumatology After Musculoskeletal Injections Of Knee Osteoarthritis With Qi Stagnation And Blood Stasis Syndrome

Objective:1.All the inpatients who was diagnosed with Knee Osteoarthritis with Qi stagnation and blood stasis syndrome from the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine from January 2017 to June 2020 by HIS system and Medical Duyun system.2.To systematically evaluate the efficacy of combinations about Ye’s Tuina of Traumatology and other therapeutic method after Musculoskeletal Injections of Knee Osteoarthritis with Qi stagnation and blood stasis syndrome.Method:1.We retrieved the medical records,collected relevant information,and organized all the scores of VAS,WOMAC,KOA quantification scale of symptom which were assessed before and after treatment.We interviewd the patients by telephone when they finished the treatment after 3 and 6 months.2.Based on the summary of data and information,the patients were divided into the following six groups according to different treatment combinations after Musculoskeletal Injections: group A(Musculoskeletal Injections + analgesic therapy),group B:(Musculoskeletal Injections + analgesic therapy + acupuncture therapy +physiotherapy),group C(Musculoskeletal Injections + analgesic therapy + acupuncture therapy + physiotherapy + Ye’s Tuina of Traumatology),group D(Musculoskeletal Injections +acupuncture therapy + physiotherapy + Ye’s Tuina of Traumatology),Group E(Musculoskeletal Injections + acupuncture therapy + physiotherapy),Group F(Musculoskeletal Injections).3.All the scores were statistically analyzed at 4 time points(before treatment,immediately after treatment,3 months after treatment,and 6 months after treatment).To observe the effect of Ye’s Tuina of Traumatology after Musculoskeletal Injections of Knee Osteoarthritis with Qi stagnation and blood stasis syndrome,we compared and discussed the difference of curative effect among several interventions.Result:1.There were 323 cases involved in this study,with males accounting for 20%(63persons)and females accounting for 80%(260 persons).The main age groups were younger old-aged people and middle-aged people,accounting for 57%(184 persons)and 29%(93persons).Among them,overweight patients reached 48%(156 persons)and obese patients reached 33%(105 persons).The incidence is more common in patients with bilateral disease,with 68%(219 people).The distribution of the tender points in the left and right knees was similar.The locations of common tender points were anserine bursa,the medial space of the knee joint,and the knee joint dial method 4 points to 8 points.2.Research result(1)Comparing with the scores before the treatment,the scores of VAS,WOMAC,KOA quantification scale of symptom were significantly declined after the treatment,the differences were significant on the statistics(P < 0.01).(2)After treatment immediately compard with 3 months later in each group,the results showed that:(1)VAS scores at 3 months after treatment in group A,B and C were significantly lower than those immediately after the treatment;the VAS score of group D and E was significantly higher than that of group B at 3 months after treatment(P<0.05),and there was no significant distinction among the other groups(P>0.05).(2)WOMAC score of group C,D and E at 3 months after treatment was significantly higher than that after treatment immediately(P< 0.01),and the score in the midst of other groups was no significant difference(P>0.05).(3)Classification and quantification score of symptom at 3 months after treatment in group A and B was significantly lower than that immediately after treatment;the score after treatmengt in group D and E was significantly higher than 3 months later(P<0.01),and there was no visible difference in other groups(P>0.05).(3)The results showed that the score of KOA quantification scale of symptom,VAS and WOMAC at 6 months after treatment were significantly higher than those at 3 months after treatment(P<0.01).(4)The comparison among the scores which were measured before and after the treatment showed that there were significant difference in VAS,WOMAC and KOA quantification scale of symptom(P<0.01).After pairwise comparison,the scores of VAS in group A was lower than that in group C,which has statistical significance(P=0.003).The scores of WOMAC in group C was higher than that in group D significantly(P=0.008).The scores of KOA quantification scale of symptom in group A,group D and group F were lower than the scores in group C(P<0.05),and the difference of classification and quantification in groups B,D and E was significantly higher than that in group A(P<0.01).(5)The comparison among the scores which were measured immediately after treatment and 3 months after treatment showed that there were significant difference in VAS,WOMAC and KOA quantification scale of symptom(P<0.01).After compared in pairs,the VAS score in group A was obviously higher than group B,C,D,E and F(P<0.01),and the variance of VAS score in group B and C was visibly higher than that in group D and E(P<0.05).The scores of WOMAC in Group E comparing with that of other groups were significant diffrernce(P<0.01),and the difference of WOMAC score in group A and B was evidently higher than that in group D and F(P<0.05).The difference of KOA quantification scale of symptom in group A,group B and group C was higher than that in group D and group E significantly(P<0.01).The score in KOA quantification scale of symptom between group C and F was distinctly lower than that in group A(P<0.05).(6)3 months after treatment and 6 months after treatment scale score comparison among the groups,the results indicated that:VAS,WOMAC and KOA quantification scale of symptom score had significant variation(P<0.01).After compared in pairs during all the groups,the difference of VAS score in group D was distinctly higher than that in groups A and B(P<0.01).The scores of WOMAC in group A was lower than that in group C and group E(P<0.01),and it in group D was also higher than groups A,B,C,E and F(P<0.05);although there was difference in the score of KOA quantification scale of symptom,the difference was not significant after correction by pairwise pairing test(P>0.05).Conclusion:1.The short-term effect of Musculoskeletal Injections combined with other therapeutic method on Knee Osteoarthritis with Qi stagnation and blood stasis syndrome is significant and stable.Ye’s Tuina of Traumatology and other therapeutic method after Musculoskeletal Injections of Knee Osteoarthritis with Qi stagnation and blood stasis syndrome.2.Among the different therapeutic method,the application of analgesics has the most significant effect on the curative effect,but the curative effect is maintained for a short time,so the clinical use should be selected according to the situation of patients.3.In the comparison of different interventions after Musculoskeletal Injections of Knee Osteoarthritis with Qi stagnation and blood stasis syndrome,"Ye’s Tuina of Traumatologyt" has a better curative effect,maintaining the best therapeutic effect after treatment.At the same time,this intervention is safe and worthy of a further study and popularization.
