Objective:To observe the clinical effect of "Xingnao Kaiqiao" acupuncture method on motor dysfunction and muscle tension recovery in patients with acute ischemic stroke, and to evaluate the improvement of "Xingnao Kaiqiao" acupuncture 、 "Xingnao Kaiqiao" acupuncture combined with Physical therapy、PT on early motor function recovery、muscle tension and overall syndrome and curative effect of stroke in patients with ischemic stroke. It is expected to provide some reference for the treatment of limb motor function and dystonia after acute ischemic stroke by acupuncture or rehabilitation training in the future.Method:A randomized controlled trial design was used in this study. Methods: 60 patients with lower extremity motor dysfunction after acute stroke were randomly divided into experimental group, control group 1 and control group 2,with 20 cases in each group. The three groups were treated with routine drug therapy for stroke. The experimental group was treated with "Xingnao Kaiqiao" Acupuncture on the basis of the treatment, and the control group 1 was treated with "Xingnao Kaiqiao" acupuncture combined with Pt therapy on the basis of the treatment,On this basis, the control group 2 was treated with sham acupuncture combined with Pt therapy. The three groups were treated 3 times a week for 6 weeks,18 times in total,30 minutes each time(PT: 30 minutes Pt scheme was applied immediately after acupuncture). Before and after treatment, the patients were assessed with simplified Fugl Meyer assessment scale lower extremity(sFMA-L),modified Ashworth Scale(MAS),stroke diagnosis and efficacy evaluation standard score. After treatment, the evaluation indexes were statistically analyzed and evaluated.Result:A total of 60 cases were included, and no cases fell off,all cases were completed.1 Baseline informationThere was no statistical significance in age, gender, course of disease,sFMA-L score,MAS score, stroke diagnosis and efficacy evaluation standard score among the three groups(P > 0.05),which showed comparability.2 Comparison of clinical efficacyAfter 6 weeks of treatment, the total effective rate of experimental group and control group 1 was higher than control group 2,and the clinical efficacy difference between the three groups was statistically significant(P<0.01).3 sFMA-L score comparisonIntra-group comparison: There were significant differences in sFMA-L scores among different time factors in the three groups(P<0.01).Scores in each group increased after 6weeks of treatment compared with those before and 3 weeks of treatment, and increased after3 weeks of treatment compared with those before treatment.Comparison between groups:After 3 weeks of treatment, there was statistically significant difference in sFMA-L scores among the three groups(P<0.01),there was no statistically significant difference in sFMA-L scores between control group 2 and experimental group(P>0.05),and there was statistically significant difference in sFMA-L scores between control group 1 and experimental group and control group 2(P<0.05).After 6 weeks of treatment, the difference of sFMA-L scores among the three groups was statistically significant(P<0.01).The scores of experimental group and control group 2 were both higher than that of control group 2,and control group 1had the highest score.4 MAS score comparisonThere was significant difference in MAS score among the three groups after treatment(P<0.05).There was no significant difference in MAS score between the experimental group and the control group 1(P>0.05).There was significant difference in MAS score between the experimental group and the control group 2(P<0.05).There was significant difference in MAS score between the control group 1 and the control group 2(P<0.05).5 Comparison of diagnosis and curative effect evaluation of strokeIntra-group comparison: there were statistically significant differences in stroke diagnosis and efficacy evaluation standard scores at different time points in the three groups(P<0.01).Scores in each group after 6 weeks of treatment were lower than those before and 3weeks of treatment, and those after 3 weeks of treatment were lower than those before treatment.Comparison between groups: After 3 weeks of treatment, scores of the three groups showed statistically significant difference(P<0.01),but there was no statistically significant difference between the test group and control group 1(P>0.05),scores of the test group and control group 1 were lower than control group 2.After 6 weeks of treatment, there was statistically significant difference in scores among the three groups(P<0.01),while there was no statistically significant difference between the experimental group and the control group 1,control group 2 and the experimental group(P>0.05),but there was statistically significant difference between the control group and the control group(P<0.05).Conclusion:1 “Xingnao Kaiqiao” acupuncture combined with Pt therapy can effectively improve the limb motor dysfunction, delay the increase of muscle tension, and improve the overall syndrome and curative effect of stroke.2 “Xingnao Kaiqiao” acupuncture is better than sham acupuncture combined with Pt in improving limb motor dysfunction and delaying the increase of muscle tension, but PT is not inferior to acupuncture in improving the overall syndrome and curative effect of stroke after 6weeks of treatment.
