ObjectiveThis study conducted symptom management education for breast cancer patients during chemotherapy,and explored the effect of symptom management education on the fatiguedepression-sleep disturbance symptom cluster,self-management efficacy and quality of life of breast cancer patients during chemotherapy.MethodsA total of 90 breast cancer patients during chemotherapy after surgery who were hospitalized in the Department of Oncology of a tertiary hospital in Tianjin from July to December 2020 were selected.By tossing a coin,patients in one of the wards were regarded as the intervention group(n=45),and patients in the other ward were regarded as the control group(n=45).Patients in the intervention group received symptom management education intervention for 4 cycles of chemotherapy on the basis of routine health education.The intervention was divided into three stages: information guidance,belief strengthening,and behavior guidance.During the patient’s hospitalization,the intervention will be one-to-one explanations at the bedside,once per chemotherapy cycle,4 times in total,30 to 45 minutes each time.During the patient’s stay at home,the intervention will be private chat on We Chat,2 times per chemotherapy cycle,8 times in total,20-30 min each time.Patients in the control group received routine health education,including one routine health education and two telephone follow-up per chemotherapy cycle.Data were collected before the intervention(the day of the second chemotherapy),the day of the fourth chemotherapy and the day of the sixth chemotherapy in the two groups.Evaluation indicators are the severity of fatigue,depression and sleep disturbance symptoms,self-management efficacy and quality of life.Statistical analysis uses SPSS21 software.Statistical methods include two independent sample t-tests,chisquare test,rank sum test,repeated measures ANOVA,and generalized estimated equation.ResultsIn the end,1 participant was dropped in the intervention group and 1 participant in the control group.There were 44 effective participants in each group,totaling 88 participants.(1)Before the intervention,the general information,the CFS score,the CES-D score,the PSQI score,the SUPPH score,the QLQ-C30 score of the two groups of patients were no statistically significant difference(P>0.05).(2)The between-group effects on the scores of total fatigue,physical fatigue and cognitive fatigue were statistically significant(P<0.05);The effect of time on the scores of total fatigue,physical fatigue,emotional fatigue and cognitive fatigue were statistically significant(P<0.05);There was an interaction effect between the group factor and the time factor of the scores of total fatigue,physical fatigue,emotional fatigue,and cognitive fatigue(P<0.05).(3)The between-group effects on the scores of total depression,depression emotion and physical symptoms of the two groups of patients were statistically significant(P<0.05);The effect of time on the scores of total depression,depression emotion,physical symptoms,and positive emotion were statistically significant(P<0.05);There was an interaction effect between the group factor and the time factor of the scores of total depression,depression emotion,physical symptoms,positive emotion(P<0.05).(4)The between-group effects on the scores of total PSQI,sleep quality,time to fall asleep,and sleep efficiency dimensions of the two groups of patients were statistically significant(P<0.05);Except for the hypnotic drug dimension score,the time effects of the scores of total PSQI and the other 6 dimensions such as and sleep quality were statistically significant(P<0.05);Except for the hypnotic drug dimension score,there were interaction effects between the group factor and the time factor in the scores of total PSQI and the remaining 6 dimensions such as and sleep quality(P<0.05).(5)The between-group effects and the time effect on score of self-management efficacy of the two groups of patients were statistically significant(P<0.05);There was an interaction effect between the group factor and the time factor of the score of self-management efficacy(P<0.05).(6)The between-group effects on the scores of total quality of life,physical function,emotional function,and cognitive function of the two groups of patients were statistically significant(P<0.05);The effect of time on the scores of total quality of life and all dimensions were statistically significant(P<0.05);There was an interaction effect between the group and the time factor of the scores of total quality of life and all dimensions(P<0.05).ConclusionSymptom management education can reduce the severity of the fatigue-depression-sleep disturbance symptom cluster,and improve the self-management efficiency and quality of life of breast cancer patients during chemotherapy.
