ObjectiveObjective to reveal the biomechanical mechanism of three kinds of lumbar vertebra pulling methods,which are oblique pulling method of massage,rotation positioning pulling method of sitting position lumbar vertebra and prone position lumbar vertebra stretching pulling method,to provide reference for clinical scientific application of pulling method.MethodsNinety patients with lumbar disc herniation who met the inclusion and exclusion criteria were randomly divided into three groups by random number table method,with 30 cases in each group: lumbar oblique pulling group,lumbar positioning rotation pulling group and lumbar draft pulling group in prone position.Average electromyography(AEMG),Median frequency(MF),VAS score and range of motion of lumbar spine were recorded in the equilength load test of erector spinal muscle and multifidus muscle of their own weight before and immediately after treatment in the three groups.The intra-group and inter-group comparisons and before and after treatment were made,and the data were analyzed using SPSS 23.0 software.Results1 Before treatment,the AEMG and MF values of erector spinal muscle,multifidus muscle in the affected side were lower than those in the healthy side.After treatment,the AEMG and MF of the erector spinal muscle,multifidus muscle of the affected side increased in the three groups compared with before treatment(P < 0.05),The values of AEMG and MF in sitting lumbar rotation and position rotation and prone lumbar drafting and rotation group were higher than those in oblique lumbar rotation and position rotation group(P<0.05),There were no significant differences in AEMG and MF between the sitting lumbar rotation and position rotation and position traction and rotation group and the prone lumbar stretching and rotation group(P > 0.05).It is suggested that the three manipulations can improve the recruitment ability of lumbar muscle group,improve muscle strength and anti fatigue after one treatment.The effect of lumbar positioning rotary pulling method is similar to that of lumbar traction pulling method in prone position.Both of them are better than lumbar oblique pulling method,but there are still differences with the healthy side.2 The VAS scores of the three groups after treatment were lower than those before treatment,The difference was statistically significant(P < 0.01),VAS scores in the prone lumbar stretching and pulling group were all lower than those in the sitting lumbar rotation and position pulling group(P<0.05),The VAS score of sitting lumbar rotation and position manipulation group was lower than that of lumbar oblique manipulation group(P <0.01),Suggest three kinds of manipulation can relieve pain,The lumbar stretching and pulling method in prone position is better than that in sitting position,The sitting lumbar rotation and positioning method is better than the lumbar oblique rotation method.3 The range of motion of lumbar spine increased after treatment in all three groups(bend,back extension,side to side flexion,side to side rotation)(P<0.05),Compared with the same position in the lumbar oblique pulling group,There were statistical differences in the flexion and dorsiflexion angles of the lumbar stretching group in the prone position(P <0.05),Compared with the other two groups,there was statistical difference between the sitting lumbar spine rotation and localization(P<0.05),It is suggested that all three manipulations can improve the range of motion of lumbar spine,The lumbar stretching and pulling method in prone position is better than that in sitting position,The sitting lumbar rotation and positioning method is better than the lumbar oblique rotation method.ConclusionThree kinds of pulling method can immediately relieve lumbar muscle fatigue,lumbar pain,enhance lumbar muscle strength,Improve lumbar range of motion.The effect of lumbar stretching and rotation in prone position and sitting position is better than that of lumbar oblique pulling in increasing muscle strength and improving psoas fatigue,The effect of lumbar stretching and pulling in the prone position is similar to that of lumbar rotation and positioning in the sitting position.The effect of lumbar stretching and pulling in the prone position is better than that of sitting lumbar rotation and position pulling,and the effect of sitting lumbar rotation and position pulling is better than that of oblique lumbar pulling. |
