Objective: This study aimed to explore the safety,effectiveness and economics of tigecycline original and generic drug by combining evidence-based evaluation and real-world research,with a view to providing reference for clinical drug selection,assisting clinical decision-making,and promoting clinical rational use of drugs.Methods: In this study,through the collection of RCT studies for the treatment of acinetobacter baumannii infection that included Tygacil,Zetan,and Sulperazon as intervention measures,with the help of bayesian network meta-analysis research methods,compared the difference in the effectiveness and safety of Tygacil,Zetan,and Sulperazon when they used alone or in combination and calculated its rank probability to indirectly compare the effectiveness and safety of Tygacil and Zetan in the literature data.Secondly,though the hospital information syetem,we can find those patients who had used Tygacil or Zetan in a large tertiary hospital,and then retrospectively collected their information on their clinical treatment.After that,we divided them into Tygacil group and Zetan group for research,and adjusted the differences between groups with the help of PSM analysis method in order to ennsure that the two sets of data are comparable.And then compared the efficacy and safety differences between Tygacil and Zetan group.Finally,based on the comparison results of effectiveness and safety in the real world,the economics of Tygacil and Zetan are compared through cost-minimization analysis.Results: firstly,according to the Bayesian Net Meta analysis,the economic research results showed that the ranking results of the total effective rate were Zetan and Sulperazon = Tygacil and Sulperazon > Tygacil = Zetan = Sulperazon;the ranking results of bacterial clearance rate were Zetan and Sulperazon = Tygacil and Sulperazon > Zetan = Sulperazon;the ranking results of WBC level after treatment were Zetan and Sulperazon = Tygacil and Sulperazon > Tygacil = Zetan ≥ Sulperazon;the ranking results of PCT level after treatment were Tygacil and Sulperazon > Zetan and Sulperazon > Sulperazon > Zetan;the ranking results of the CRP level after treatment were Tygacil and Sulperazon ≥ Zetan and Sulperazon > Zetan ≥ Sulperazon.The results of the safety research showed that the frequency of adverse reactions were Zetan >Zetan and Sulperazon > Sulperazon > Tygacil + Sulperazon > Tygacil;the results of the incidence of nausea and vomiting were Zetan and Sulperazon ≥ Zetan > Sulperazon >Tygacil + Sulperazon > Tygacil;the results of the incidence of rash were Zetan ≥ Zetan and Sulperazon > Sulperazon > Tygacil + Sulperazon.Secondly,real-world studies showed that the results of effectiveness analysis were that 72 h cure rate,cure rate at discharge,bacterial clearance rate,body temperature,CRP,PCT and other indicators were not statistically significant in the Tygacil group and the Zetan group after PSM.The results of the safety research were that the incidence of adverse reactions in Zetan group was higher than that in the Tygacil group,but there were no differences in the indexes of liver,kidney and blood system.The incidence of rash and diarrhea in Zetan group were higher than that in Tygacil group,and there was no statistical difference in other adverse events.Thirdly,the cost-minimization analysis method suggested that the Zetan group is more economical.Conclusion: there is no difference between the effectiveness of Zetan and Taige,but the safety of Zetan is lower than that of Tygacil.And in general Zetan is more economical than Tygacil.
